CAPMC is seeking a strategic and creative person responsible for providing homeless clients with direct case services including advocacy, support, goal setting, assistance in identifying options, evaluation of needs and information, and making necessary community linkages to support their self - sufficiency goals.
The individual will demonstrate the ability to:
- Communicate orally and in writing in Spanish is required.
- Use Microsoft Outlook, Teams, 365 products.
- Communicate effectively orally and in writing with individuals and groups.
- Work effectively under conditions of limited supervision, high stress and rapidly changing situations and circumstances.
- Demonstrate sensitivity to the generation / cultural / ethnic diversity of the service population.
- Have excellent documentation/organizational skills.
- Supervise and provide training in a structured setting.
- Work as a team with other CAPMC employees.
EDUCATION / EXPERIENCE
- Bachelor’s Degree in Social Work, Psychology, Counseling or related field.
- One-year experience working in a similar or related field or equivalent of education/experience substitute.

GENERAL DUTIES:
- Participates in training set forth by the Fresno Madera Continuum of Care (FMCoC).
- Screens and evaluates clients to prioritize enrollment.
- Establishes professional relationships with property management companies and real estate agents/owners for the benefit of clients.
- Works with the FMCoC Homeless Outreach teams to help seek out high-risk population in targeted areas.
- Works with the FMCoC’s Coordinated Entry System Entity to ensure enrollments meet requirements set forth by the FMCoC.
- Engages homeless individuals by conducting Vulnerability Index Specialized Data Assessment Tool (VI-SPDAT) Assessments.
- Seeks and maintains necessary community linkages to support identified needs of clients.
- Collects and documents data as required, and assists in compiling statistical data, Housing Management Information Systems (HMIS) and monthly reports.
- Attends training, meetings and other grant-sponsored events both in and out of the county.
- Participates in community events such as the Homeless Point in Time Count, program, etc.
CASE MANAGEMENT:
- Works with clients to address barriers to housing.
- Provides information, referrals, linkages, and advocacy to assist clients in accessing services and resources.
- Assists clients with procuring necessary documents and services such as identification cards, birth certificate, social security income, disability income/verification, certification of homelessness, and other documents as needed.
- Participates in case-conferencing related to client work and progress.
- Once a housing match is made, work with Housing Matcher to identify appropriate permanent housing options for clients.
- Assists clients with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing, and advocate for clients with prospective landlords.
- Manages a caseload to get clients documents ready and identify housing options.
- Works with clients to help develop goals.
- Provides ongoing support and conflict resolution for clients.
- Responsible for referring individuals to appropriate community resources for additional assistance and counseling.
- Provides advocacy and support in necessary meetings for clients (medical appointments, court, Social Security Benefit meetings, etc.)
- Provides transportation when necessary and safe.
- Enter Information into HMIS (Homeless Management Information Systems)
GENERAL PHYSICAL REQUIREMENTS
- Exerting up to 15 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
- Able to maintain regular, punctual attendance consistent with the Americans with Disabilities Act (ADA), Family Medical Leave Act (FMLA), California Family Rights Act (CFRA) and other federal, state and local standards.
- Able to perform the essential job functions consistent with the ADA, FMLA, CFRA and other federal, state and local standards.
