What are the responsibilities and job description for the RCP & Homeless Services Worker position at Community Action Partnership of San Luis Obispo County, Inc.?
As part of the Recuperative Care Team at 40 Prado, you will support homeless individuals who need 24-hour supervision after being discharged from hospitals or nursing facilities. Your main responsibilities include collaborating with the team and other shelter staff to provide essential services that stabilize clients' physical health. You will assist clients with daily living activities to help them maintain their physical health and work alongside emergency services personnel, such as law enforcement and paramedics. Upholding CAPSLO’s mission, you will offer care and assistance to clients, promoting their journey towards self-sufficiency. This role is crucial in ensuring that clients receive the necessary support to recover and transition smoothly back into the community.
Qualifications:
Has a combination of four years of education and/or experience providing direct human or health services to individuals in need, including those who are economically disadvantaged, physically/mentally disabled, elderly, or at-risk. Volunteer experience may also be accepted.
Has a strong desire to help those in need, a working understanding of issues surrounding homelessness, and a commitment to CAPSLO’s mission of promoting self-sufficiency.
Has the ability to work closely with other staff, volunteers, and service providers to deliver coordinated care and services.
Must have dependable, insured transportation, and a valid California Driver License (mileage to be reimbursed) and acceptable driving record. A DMV printout and proof of insurance will be required.
As a Recuperative Care Homeless Services Worker You Will:
Recuperative Care: Work with a team of healthcare professionals to assist clients with activities of daily living and support their physical health recovery.
Shelter Care: Provide direct services to homeless individuals and families, including assessment, information and referral, and distribution of basic supplies.
Documentation & Coordination: Help manage client documentation, logs, and reports while working closely with multiple service providers to enhance client care.
Health & Safety: Ensure a safe, clean environment for clients, coordinate food services, and maintain proper sanitation practices.
