Under the general supervision of the SLOHub Supervisor and/or the Clinical Program Manager & Homeless Services Director, the Peer Support Specialist assists the SLOHub Case Manager in assisting clients in developing and achieving goals with chronically homeless adults with co-occurring, mental health and substance use disorders. This role is critical for individuals to connect to determine the best client-centered pathway out of homelessness. Through this mentorship, the peer support specialist will encourage participants to take an active role in obtaining secure and stable housing and attending SLO HUB treatment. Trusted to guide and mentor participants towards self-sufficiency, the Peer Support Specialist provides encouragement, assistance, and support, fostering long-term recovery. This role supports CAPSLO’s mission by extending care and assistance to participants in their journey toward improved health and stability.

Qualifications:
- High school graduation or GED.
- Experience in navigating the day-to-day life of accessing homeless services, mental health services, physical health services, and drug and alcohol services.
- Lived experience of homelessness, and addiction and is currently in a stable housing setting.
- Dependable, insured transportation, a valid California Driver's License, and an acceptable driving record.
- Bilingual in English and Spanish preferred but not required.
- Collaborate with the SLOHub team to assist the case manager with integrated treatment plan goals.
- Help participants obtain vital documents, apply for assistance programs, and fill out important forms.
- Ensure adherence to site-specific policies and protocols.
- Assist with the intake of new participants and manage the sign-in and data collection process.
- Provide referrals to available community resources.
- Oversee participants' assigned care plan duties.
- Utilize prevention measures, conflict resolution skills, and crisis management techniques.
- Complete necessary documentation accurately and timely.
- Participate in staff meetings and training sessions.
- Provide emotional support, guidance, and advocacy for participants facing challenges in their recovery.
- Facilitate support groups, individual coaching, and offer empathetic listening and coping mechanisms.
