What are the responsibilities and job description for the Healthcare Navigator position at Community Action Region VI?
Healthcare Navigator to Provide services statewide to connect veterans to VA healthcare benefits or community healthcare services throughout the state. Work with veterans on a variety of issues to assist them in identifying and overcoming challenges to accessing the healthcare system or adhering to recommended health care plans. This position is a key member of the CAPND SSVF leadership team and is expected to contribute to the overall success of the program.
Qualifications :
- Four-year degree in a human service-related field, or equivalent combination of experience and education.
- Knowledge of health care systems and coordination
- Ability to operate basic office equipment
- Excellent written and oral communication skills
- High level of integrity, ability to maintain confidentiality and deal with sensitive information in a professional manner.
- Ability to work independently with general direction from supervisor.
- Must possess a valid driver’s license and be able to travel independently.
This position is full-time year-round, Monday through Friday, with full benefits.
