Community Alliance, offering an integrated health care approach and built on an array of mental health and primary care services, is seeking a Care Coordinator. Ideal candidates will have a passion for assisting individuals who are reentering after being incarcerated..
Days - 40 hours: 8:30am-5:00pm Mon-Fri
Maintain and utilize current knowledge, information, and best practices related to community-based behavioral health services and integrated care, inclusive of motivational interviewing, trauma-informed care, recovery model, person-centered care, community resources, and health home model of integrated care.
Maintain and utilize current knowledge, information, and best practices related to community-based behavioral health services and integrated care, (e.g. knowledge of medications, community resources, strengths-based assessment, motivational interviewing, trauma-informed care, cultural competency, co-occurring mental illness and substance use disorders, person and family centered care, philosophy and principles of recovery).
Demonstrate skills necessary to consistently serve as effective advocate for homeless persons in the community and while representing or associated with the organization.
Assume responsibility and successfully complete all training as outlined in organizational policy.
Initiate a professional development and training plan, identifying professional needs, development objectives, and timelines, and complete on at least an annual basis.
Demonstrate and maintain functional computer skills, including orientation to privacy and security standards, internet usage and use of electronic health record.
Earned Bachelor’s degree in social work, rehabilitation, education, or other relevant field from an accredited academic institution; or current certification as a peer support specialist in the State of Nebraska.
Demonstrated knowledge and skills in mental health rehabilitation and recovery principles and behavior intervention/management practices on direct service level.
Demonstrated knowledge of characteristics and needs of persons with a serious mental illness, including persons with co-morbid medical conditions..
Demonstrated knowledge of available behavioral health services, social services and other community resources and basic case management principles.
Ability to maintain effective working relationships with individuals of diverse backgrounds including persons with mental illness, behavioral health and physical healthcare professionals, family members, social service providers, and public at large.
Ability to connect, communicate, and effectively work with people from diverse cultural, ethnic, socio-economic, educational, and professional backgrounds; and, ability to maintain effective working relationships with diverse individuals and groups.
Ability to initiate and complete tasks in areas of responsibility with moderate to minimal supervision.
Demonstrated ability to effectively communicate both orally and in writing.
Ability and willingness to work varied hours, including evening and weekend hours as required by consumer need and/or assigned.
Ability to transport self to and from community sites to perform position responsibilities including possession of valid driver’s license, good driving record, and reliable transportation or the ability to arrange for such on a regular basis
