We are Community Alliance, an integrated health care organization that offers a whole-person approach, including psychiatrics services, mental health and substance use counseling, primary medical care and a range of rehabilitation, employment, community support and family and peer support.

We are seeking an Emergency Community Support Specialist. Ideal candidates will have a passion for working with vulnerable populations and enjoy collaborating with other professionals to empower clients to achieve self-defined goals.

The Emergency Community Support Specialist is responsible for collaborating with local Emergency Departments and Hospitals in the Omaha area with the goal of reducing the need for emergency room and hospital visits by educating, administering and delivering mental health services, case management, and identifying community resources for adults with serious mental illness. This position will be full time with availability for days and some nights and/or weekends.

Initiate, engage and meet with individuals with mental health at Emergency Department or Hospital prior from discharge or shortly after discharge to conduct screenings and assessments to determine the individual's needs.

Assist with the development, revision and coordination of a plan of care with services that meet the client’s needs through various collaborations with others; assist the client in accessing and utilizing the needed services and supports. Typically 90 days.

Facilitate assessment of client and involved family’s unmet health and social needs; educate about relevant community resources and facilitate/coordinate referral, access, and utilization of such resources; serve as the client and family member’s contact of point, advocate and informational resource.

Actively assist and promote client development and utilization of the Safety/Crisis Plan and wellness self-management goals.

Facilitate client access to appropriate behavioral health, medical and any needed specialty providers.

Assist individuals in making and remembering appointments for acute, preventative, and follow-up care, and assist clients in keeping their scheduled appointments.

Actively foster health literacy knowledge and skills, and assist persons served in effective communication and self-advocacy with primary and behavioral health providers.

Provide instruction and supports to persons served regarding maintaining their self-medication regimen, including picking up their medications from the pharmacy and taking their medications as prescribed.

Practice motivational interviewing strategies and techniques throughout the assessment, individual program planning, and service delivery process and actively promote and facilitate client self-determination.



  • Bachelor’s Degree in Psychology, Sociology or Human Services related field, or two years post High School course work in a Human Services field, plus two years relevant experience/training of a professional nature
  • Demonstrated knowledge and skills in mental health and substance abuse rehabilitation and recovery principles, service delivery, and behavior intervention/management practices on direct service level.

  • Basic knowledge of characteristics and needs of persons with serious mental illness, including persons with co-occurring mental illness and substance use disorders.

  • Ability to maintain effective working relationships with diverse individuals and groups.

  • Possession of valid driver's license and good driving record.
  • Will also consider a Peer Specialist with two (2) years lived experience and will require a state certification within six (6) months of hire.
