Community Alliance is offering an Intermediate Psychiatric Residential Services LPN to work in one of our residential locations to provide medication assistance, education on personal physical health/wellness and follow-up with health care providers. Persons served are individuals who need a structured recovery environment to reduce hospitalizations and increase independent living.
Ideal candidates will have a passion for working with vulnerable populations and be comfortable in collaborating with other professionals to empower clients to achieve self-defined goals. The Licensed Practical Nurse (LPN) is responsible for increasing the visibility, linkage, and impact of the organization’s integrated healthcare efforts throughout the agency with a particular emphasis on the agency’s community rehabilitation services.
An individual employed in this position works as a member of a collaborative team and is responsible for the provision of direct and collateral services, including group facilitation and individual teaching and coaching which promote skill development and personal recovery for persons with serious mental illness and co-occurring mental illness and substance use disorders. An individual employed in this position is also responsible for maintaining a safe, supportive, and rehabilitative environment for persons participating in our program.
Develop, coordinate, and conduct healthcare screenings within the Intermediate Residential program.
Conduct health literacy classes
Design and facilitate health-related educational and experiential learning opportunities relevant to the individual and population-based needs of persons served (e.g. nutrition, exercise, diabetes management).
Assist in the continuing development and enhancement of agency-wide tobacco cessation and other wellness initiatives based on evidence-based practices.
Provide support to other team members in their area of expertise to increase inter-professional staff knowledge and effectiveness.
Contributes as a member of the rehabilitation and integrated health treatment team and care plan.
High school diploma or GED and graduation from an accredited LPN program required.
Licensed in Nebraska as LPN required.
Demonstrated knowledge of characteristics and needs of persons with a serious mental illness, including persons with co-occurring mental illness and substance use disorders.
Ability to maintain effective working relationships with individuals of diverse backgrounds including persons with serious mental illness and co-occurring mental illness and substance use disorders, family members, behavioral health professionals, government representatives, and the public at large.
Demonstrated ability to effectively communicate both orally and in writing.
Possession of valid driver’s license and good driving record, or ability to arrange for reliable transportation.
