Manager, Healthy Transitions Program in our Integrated Health Clinic:
Community Alliance has an exciting opportunity, providing leadership in the development and implementation of a new program initiative, Healthy Transitions, targeted towards transition age youth and young adults (ages 16 to 25) in Douglas and Sarpy counties who have or are at risk for developing a serious emotional disturbance or serious mental illness. In collaboration with key community partners including those serving justice-involved youth and young adults, the program will provide outreach, system coordination, and direct service aimed at helping maximize the potential of this population to assume adult roles and responsibilities and lead full, productive lives.
Duties for Manager, Healthy Transitions Program in Integrated Health Clinic:
- Facilitate and lead Healthy Transitions leadership team involving key community partners including local behavioral health, state/local youth-serving justice authorities, and other community organizations.
- Implement multi-year plan for filling in service and care coordination/navigation gaps for persons served including needed training to support the development of services and coordination activities.
- Develop and oversee reliable referral pathways to evidence-based practices and other services within the youth-serving and adult-serving behavioral health systems, including for youth aging out of child/youth systems.
- Provide outreach, care coordination, and direct services to population of focus utilizing evidence based and team approaches effectiveness such as Coordinated Specialty Care (CSC) for transition-aged youth with first episode psychosis and Critical Time Interventions.
- Assure coordination of program activities with existing Community Alliance services for purposes of maximizing service effectiveness and continuity of care for population of focus.
- Initiate and continuously engage a well-informed advisory council comprised of at least 51% youth, young adults, and family members.
- Supervise, train and evaluate program staff.
- Work collaboratively with independent evaluator, project staff, community partners, and Community Alliance leadership to evaluate and analyze project activities.
- Communicate and coordinate regularly with federal grant officials; submit all required data and reports in a timely manner.
- Possession of current, valid driver’s license, reliable transportation, and good driving record.
Qualifications for Manager, Healthy Transitions Program for Integrated Health Clinic
- Earned Masters degree in counseling, social work, public health, healthcare administration, public administration or other relevant field.
- Minimum 5 years progressively responsible experience in healthcare or behavioral healthcare planning, management, service delivery, systems change, and/or quality improvement.
- Licensure in the State of Nebraska required as (a) Mental health professional (LIMHP), (LMHP) or provisional licensure (PLMHP) required.
- Successful supervisory experience.
- Demonstrated competencies in the delivery of person-centered care, recovery and collaborative care models; trauma informed care; knowledge of community resources and effective service delivery within adolescent and/or adult service systems.
- Demonstrated cultural and linguistic sensitivity and competence.
- Knowledge, training, and/or experience in effectively working with individuals with serious mental illness, substance use disorders, co-occurring disorders, and co-morbid physical conditions.
- Excellent written and verbal communication skills.
- Ability to connect, communicate, and effectively work with people from diverse cultural, ethnic, socio-economic, educational, and professional backgrounds, and ability to maintain effective working relationships with diverse individuals and groups.
