What are the responsibilities and job description for the RN - Safe Haven position at Community Care Alliance?
Safe Haven offers a set of supportive services for individuals who are unhoused and actively using substances, often with co-occurring mental health challenges.The Safe Haven team is a multidisciplinary team offering psychiatry, nursing, case management, peer recovery support, hospital liaison, and housing support.The primary focus of this position is care coordination and addressing the social determinants of health to improve health and well-being. The nurse is integral to this goal, working one-on-one with clients to remove barriers to care. The nurse must be willing to work with clients in their homes and in the community.The Support includes:Assessment of health issues and development of an individualized plan to adhere to medical recommendations and improve healthPrimary care coordination,Support the development of healthy habits related to sleep, diet, exercise, etc.Building an improved understanding of chronic health conditions and strategies to manage these.Coordinate client access to pre-packaged medications when necessarySchedule and sometimes attend medical appointments with clients and confirm client understanding of findings and follow-upAdminister injectable antipsychotic medications, sometimes in the context of court-ordered treatmentMaintain availability in early evenings (occasionally/ as needed) to meet the needs of employed clients/ family membersConsult/liaison with psychiatrist and/or medical doctors and other treatment staff;Provide referral consultation and education to the community regarding issues of chemical dependency and mental health.Timely documentation according to agency standards and state regulations.
