The Opportunity and Responsibilities │ Job Summary
Provides integrated health screenings, assessment, diagnosis, evidence-based treatment, case management and comprehensive psychosocial services (as required) to patients of our clinic. Most of our patients are Spanish speaking. Applicants must be fluent in Spanish and English, as well as be aware and knowledgeable about Hispanic/Latinx culture. Our clinic is in Winston Salem. The clinic is a not-for profit community clinic that offers free medical and mental health services to the populations that need it most and do not otherwise have access to services. It is a laid back, professional, yet family environment that focuses on supporting each other as we provide the best services possible to our patients. Here, you will obtain experience working with a multidisciplinary team and develop well rounded skills. No lengthy Comprehensive Clinical Assessments (CCA) required with every client. You will have flexibility with therapeutic approaches to address the patient’s individual clinical goals. We take a cultural healing approach that allows for exploration and personalization of the patient’s treatment plan. We use an Electronic Health Record (EHR) system to minimize paper files and scanning or uploading. No pressure of having to have a specific number of billing units each week or feeling micromanaged by supervision. The clinic has support staff to assist with scheduling or rescheduling patients and other tasks.
The Responsibilities │ Essential Functions
- Provides integrated care screenings, clinical behavioral health diagnoses, treatment planning, evidence based clinical interventions, information, and referrals to community.
- Provides brief psychotherapy, supportive counseling, and education to patient/patient's social support system. Assists in coping with stress related to mental health concerns and diagnosis, previously lived traumatic experiences, grief / loss, chronic illness, substance abuse, or other related factors.
- Develops or coordinates a plan of care based on assessment of patient/family needs, in collaboration with multidisciplinary team.
- Provides psychoeducation to patient/family as related to their needs.
- Advocates for the patient and family throughout care continuum.
- Maintains availability to the patient/family as a resource to facilitate communication among providers and to monitor services rendered.
- Assist in providing supervision to Graduate Students of the Master’s Level Interns within the clinical team.
- Serves as a resource to team members and physicians regarding emotional, social and psychosocial components of the patient's illness and its effects on their social support system.
- Intervenes in crisis situations professionally, appropriately, and effectively.
- Refers patients/family to external agencies/resources for treatment when appropriate.
Education and Experience:
- Master’s degree in Social Work (preferred), Clinical Counseling or related field required.
- Previous experience within integrated health is a plus but not required.
- Candidates must possess or be able to obtain one of the following licenses in North Carolina: LCSW, LCMHC, LMFT, or any Associate Level equivalent.
- Recently graduated or associate level clinicians are encouraged to apply.
Skills and Abilities:
- Documents accurately, timely and according to departmental policy.
- Strong computer literacy skills are necessary for the role.
- Accommodate patient’s meeting location in very rare cases, as needed based on patient mobility or medical needs.
- Strong interpersonal, written, and oral communication skills
- Adequate hearing, verbal and visual abilities in order to maintain communication with patients, as well as assess mood/affect and behavioral aspects of the patient’s clinical state.
- Ability to work independently and collegially with a range of individuals, volunteers, providers, internal and external stakeholders
- Enthusiasm for medical, case management and mental health access for all.
Equipment and Programs Used:
Working Conditions/Hours: In person, typical Medical community office environment and primarily indoors. Salary position based on an average of 40 hours per week.
Physical and Mental Requirements: Traditional requirement of sitting, standing, walking throughout the day.
