- Shift : NOC 11pm-7am wth an every other weekend rotation
- Must be able to attend a daytime orientation at 8am

Community Care Center is a large inner-city Skilled Nursing Facility. We have patients of all ages with multiple levels of care, including long term care and a large behavioral health (psych) population.
The National Association of Rehab Providers & Agencies ( NARA ) awarded Transitional Care Management their “Impact Award” for 2023 for our Life Skills Programming. As a Licensed Practical Nurse (LPN), you will be joining a team of nursing professionals dedicated to providing excellent clinical care. Some have been with us for a long time and can mentor others. You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
