BASIC FUNCTION: 

The provision of quality authorized services, in a variety of settings, to program participants with intellectual/developmental disabilities, at the staff-to-program participant ratio authorized, not to exceed 1:6

NATURE AND SCOPE: 

This position reports to the Manager of Adult Services- Day Services.  This position interacts daily with program participants in a variety of settings including integrated community locations, our CSP Center (Golden Opportunities Center), other segregated settings and events, and program participants’ homes.  As a result, this position will interact with community members including those who operate facilities (such as the YMCA and public library) as well as other people accessing those locations.  Direct Support Professionals (DSPs) will interact with the program participant’s families, friends, residential staff, and/or other provider staff, including but not limited to behavior specialists, supports coordinators, and legal guardians.

DSPs provide the transportation services necessary to enable program participants to access their community for the delivery of services in accordance with their Individual Support Plan (ISP) and may require the use of DSP’s personal vehicle.  Through the provision of services, program participants will acquire, maintain, or improve skills necessary to live in the community; to live more independently; or to be more productive and participatory in community life. 

 

RESPONSIBILITIES:

  • Prioritize the empowerment, choices, rights and safety of people with disabilities
  • Work closely with Program Specialists, Behavior Support Professionals, and other DSP’s to develop and implement quality individual support plans to support growth and independence of persons enrolled in the program
  • Work independently and collaboratively to plan and provide direct service to program participants in a variety of settings to support skill development and independence
  • Provide direct service in a variety of settings to increase opportunities for and assist program participants in achieving desired outcomes as outlined in their Individual Support Plan (ISP)
  • Coach and assist, rather than do for the participants, to the extent each participant and situation allows.
  • Provide personal care and/or assistance to program participants as necessary including, but not limited to: safe transfers to and from wheelchair and personal hygiene including assistance in the bathroom
  • Complete/pass and maintain PA State Medication Administration certification and administers medications to participants as required
  • Research, suggest and provide opportunities in program participants’ own communities that align with their interests and goals
  • Develop and maintain the necessary skills to communicate with and support the program participants in their methods of communication, e.g. American Sign Language, assistive devices, etc.
  • Accurately and thoroughly complete all necessary documentation and training in accordance with CCC policies and procedures, funding stream regulations, and program participant’s Individual Support Plan
  • Report and document any observed changes in program participant’s health status /condition immediately to the Manager of Adult Services-Day Services
  • Build and maintain strong working relationships with other support staff to ensure participants’ interests, dreams and challenges are communicated to their ISP team
  • Build and maintain strong working relationships with Behavior Supports to cooperatively ensure the best possible outcomes for the participants behavior modification
  • Follow CCC policies and procedures
  • Cultivate and implement effective communication skills
  • Maintain HIPAA by following all security procedures to protect PHI and by immediately reporting any suspected or actual privacy or security incidents.
  • Immediately report any concerns, specifically those of abuse, neglect, fraud or waste
  • Utilize adaptive equipment including wheelchairs, assistive communication devices and other equipment
  • Assist program participants who have disabilities with mobility, personal care or other physical needs related to their participation in the program activities
  • Properly operate a vehicle lift, use tie downs or straps to ensure the safety of the program participant
  • Assist participants in their homes with meal preparation and housekeeping tasks.
  • Assist with preparation of meals and snacks at the Adult Day program as assigned.
  • Complete and maintain certifications as required
  • Complete housekeeping tasks as assigned
  • Other related duties as required

Qualifications

  • High School Diploma/GED
  • 1 year experience working with individuals with intellectual disabilities preferred.
  • At least 18 years of old
  • Eligible to work for CCC per the Medicaid Integrity Provisions
  • Clearances- PA Criminal History, PA Child Abuse History, FBI to comply with Act 31
  • Excellent time management and organizational skills
  • Valid PA driver’s license, insurable driving record and reliable transportation or the ability to independently travel to the program site, meetings or trainings.

  • Sit, stand, walk, bend, crouch, or stoop frequently to constantly
  • Climb stairs occasionally to frequently
  • Lift up to 50 pounds occasionally to frequently
  • Reach outward and upward occasionally to frequently
  • Lift and/or transfer adult program participants in a safe manner occasionally to frequently
