Overview
We are seeking a compassionate and dedicated Social Worker to join our family-owned hospice agency. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges. This role involves working closely with clients to assess their needs, develop treatment plans, and facilitate access to resources and services. The Social Worker will play a vital role in promoting the well-being of clients, particularly those with disabilities, children, and other vulnerable populations. Currently a contract position with the potential to transition into a salaried role based on performance and company needs.
Responsibilities
- Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
- Provide psychotherapy and counseling services to individuals and groups, focusing on mental health and emotional support.
- Collaborate with interdisciplinary team members to provide comprehensive hospice care.
- Utilize medical terminology and knowledge of ICD-10 codes in documentation and reporting.
- Advocate for clients by connecting them with community resources, including home care services and support groups.
- Facilitate individual and group counseling sessions tailored to the unique needs of clients, including those in pediatrics.
- Maintain accurate records of client interactions, progress notes, and treatment plans in compliance with legal and ethical standards.
- Stay informed about best practices in social work, particularly regarding working with people with disabilities.
Experience
- A degree in Social Work or a related field is required; licensure is preferred.
- Experience in patient care settings, particularly with individuals with disabilities or pediatric populations, is highly desirable.
- Proficiency in psychotherapy techniques and individual/group counseling methodologies is essential.
- Familiarity with medical terminology and the ability to navigate ICD-10 coding is beneficial.
- Strong interpersonal skills to build rapport with clients from diverse backgrounds.
- Ability to work collaboratively within a multidisciplinary team while maintaining client confidentiality.
