Behavioral Health Specialist

Community Care is hiring a Behavioral Health Specialist. This position covers the Waukesha and Racine PACE Programs. Behavioral Health Specialist Job Responsibilities: Responsible for providing Behavioral Health (BH) services across Community Care Inc. programs for designated members. Serves as a resource to the Interdisciplinary Team Provides targeted Behavioral Health Case Management services, as assigned. Behavioral Health Specialist Job Requirements: Experience: A minimum of 3 years of experience in providing social work and/or behavioral health case management services to older adults and adults who are physicallyand developmentally disabled required; Mental health experience, including provision of individual counseling and support group facilitation with older adults is preferred. Behavioral Health Specialist Education:Masters Degree, Social Work, Counseling or related field required. Certifications/Licensures: Required  LCSW, LPC or Psychologist. *Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Behavioral Health Specialist Job Schedule: Monday to Friday - 8:00AM – 4:30PM No weekends, major holidays, or on-call.
