Dietitian - Full-Time - Day Shift
Community Care is hiring a Dietitian to join our Community Care team. This is a full-time opportunity at our Vliet St. location.
Dietitian Job Responsibilities:
Calculates data obtained from food intake records and calorie counts, and makes recommendations for changes.
Implements the nutrition care plans for members.
Promptly reports any unusual or sudden changes in a member's condition or behavior .
Responsible for the management of therapeutic diets, including modifications of the general menu to meet individual member needs and physicians' orders.
Participates in and provides in-service education to Community Care employees and members as requested.
Documents nutrition notes in the member’s electronic medical record.
Facilitates and participates in Nutritional Improvement Program (NIP) reviews as scheduled.
Assists in monitoring meals and supplement use, including intake, diet compliance and tray accuracy.
Maintains supplement list and records for assigned site(s).
Functions as a member of a care team(s) and participates in team meetings, including care planning and service/resource allocation decisions.
Performs home visits and/or telephone consults when needed.
Position includes work in Milwaukee and Racine/Kenosha areas.
Dietitian Job Requirements:
Education: Bachelor's degree in Dietetics.
Licensure: Registered Dietitian through the American Dietetic Association or Certified through the State of Wisconsin.
Licensure: WI Drivers License
Experience: A minimum total of one year of personal, clinical/internship, and /or employment experience providing care or services for adults with disabilities and elderly population required.

Monday to Friday - no weekends and no on-call responsibilities
8:00AM – 4:30PM
