Functional Screen Manager - Milwaukee, WI
Community Care is hiring a Functional Screen Manager to join our team. This rewarding opportunity is perfect for someone who enjoys team collaboration and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule. It is based in Milwaukee County with in-home telecommute currently being the norm.
Functional Screen Manager Job Responsibilities:
• Identifies and improves functional screen processes to allow capturing of all revenue appropriate for services being provided
• Educates and communicates with staff regarding processes, auditing and reporting outcomes of the processes, and utilizing all internal and external resources in order to accomplish and sustain improvements.
• Assists with the development of protocols, policies and procedures for member functional eligibility screening to ensure accurate and timely functional screens.
• Monitors the development and implementation of corrective action plan for screeners with scores below acceptable range.• Performs targeted auditing of functional screens to determine screen accuracy and identify potential screen quality concerns and screener education needs.• Manages screen situations involving potential negative outcomes for members and the organization. Develop/Manage Long-term Care Functional Screen (LTCFS) Quality Assurance/Improvement Activities. Ensure consistency between data gathered in LTCFS and data in member’s record and care plan.
Functional Screen Manager Job Requirements:
Education: Bachelor’s Degree in a Human Services Area required.
Experience: 4 years of experience in human services delivery with 2 years of experience with older adults or with persons with physical or intellectual disabilities 1 year of supervisory experience required. .
Certification: Eligibility for long-term care functional screener certification is required
Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Functional Screen Manager Job Schedule:
• Monday to Friday
• Generally during business hours of operation (8AM-4:30PM)
