Licensed Practical Nurse, LPN Residential Care Coordinator

Licensed Practical Nurse/LPN, Part-Time, Day Shift, 0.8 - Milwaukee, WI  (Sign-On Bonus Available) Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. This is a part-time opportunity on our day shift for Sacred Heart location at 1555 S. Layton Blvd, Milwaukee, WI 53215. Licensed Practical Nurse/LPN Job Responsibilities:   Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday operations Serves as coordinator of care for scheduled weekend and holiday shifts Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.  Licensed Practical Nurse/LPN Job Requirements:   Education: Diploma from accredited program in practical nursing required Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing  Certification: Current CPR/AED certification  Licensed Practical Nurse/LPN Job Schedule:  Hours of Operation, Monday-Friday (6:30 a.m. to 3 p.m.) Work Schedule, every other weekend and every other holiday   Community Care is a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible.
