Nurse Practitioner, Full-Time, Day Shift
Community Care is hiring a Nurse Practitioner for our Pace Program. This is a full-time opportunity at our Vliet St location.

Nurse Practitioner Job Responsibilities:
Documents and updates health history and findings, physical exam, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
Within their official scope and practice, delegates appropriate tasks to interdisciplinary team staff, and provides leadership and guidance. Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance.
Educates members and families regarding health maintenance and acute and chronic illnesses.
Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Nurse Practitioner Job Requirements:
Education: Masters degree in Nursing required.
Licensure: Wisconsin Registered Nurse/RN license required. Family, Geriatric, or Adult NP certification is required. DEA registration required. Advance Nurse Prescriber required. NPI and CPR & AED required
Experience: minimum of 1 year recent clinical nursing experience with an at risk or elderly population.
Nurse Practitioner Job Schedule:
Monday - Friday, 40 hours per week
8:00AM - 4:30PM
