Rehabilitation Specialist - Full-Time - Day ShiftCommunity Care is hiring a Rehabilitation Specialist to join our team. Rehabilitation Specialist Job Responsibilities:
Under the supervision of the Rehabilitation Specialist Supervisor, and in coordination with Site and Program Managers and Regional Director of Operations, is responsible for consulting with staff to provide guidance, education and leadership regarding rehabilitation services and durable medical equipment (DME) in the Family Care and Partnership programs in Dane, Jefferson, Rock and Columbia counties.
Consults on members' need for adaptive and/or durable medical equipment (DME) and/or home modifications.
Consults on the need for on-going skilled PT/OT/ST and assists teams in developing a discharge plan.
Serves as a resource to teams in care planning, problem-solving and providing resources and interventions for members. Will develop and present training to teams including assisting with new employee orientation.
Performs clinical review of authorized skilled nursing and outpatient rehabilitation services and DME quotes.
Completes documentation of therapeutic interventions per company and regulatory requirements.
Rehabilitation Specialist Job Requirements:
Education: Required - Bachelors degree in Occupational Therapy or Physical Therapy Preferred - Masters degree in Occupational Therapy or Physical Therapy
Licensure: WI Drivers License
Experience: Required - Minimum of 3 years recent clinical experience with adults over 65, or persons with physical or developmental disability in a hospital, home care, or nursing home setting required. Preferred - Strong background in home care, case management, or other aspect of multidisciplinary care management; deep understanding of DME and seating/mobility products
Rehabilitation Specialist Job Schedule:
Monday to Friday
8:00AM – 4:30PM
