Registered Nurse/RN Program Manager, Full Time - Sacred Heart   Community Care is hiring a Registered Nurse/RN Program Manager to lead our dedicated team providing day shift nursing care and service for the community of sisters living at the Sacred Heart Convent, located in Milwaukee WI   The Registered Nurse/RN Program Manager is perfect for someone who enjoys leading while having the opportunity to provide some hands on care, team collaboration, and being part of a mission driven organization. *****Sign On Bonus $8,000.00*****   Registered Nurse/RN Program Manager Job Responsibilities: ·        Manages Community Care’s Program of All Inclusive Care for the Elderly (PACE) operations at the Sacred Heart Convent site; ensures quality and compliance of care and service delivery. ·        Works collaboratively with Leaders of the Sister’s community to ensure quality of nursing care and service delivery provided to PACE members. ·        Serves as a consultant for the interdisciplinary team on member specific issues as needed. ·        Conducts and prepares quality assurance audits/studies. ·        Works with the staff scheduler as needed to ensure continuity of member care and service. ·        Provides orientation, ongoing training and feedback to direct and matrix reports to ensure their job responsibilities and performance expectations are defined and understood. ·        Coordinates, delegates or preforms special assignments and projects.   Registered Nurse/RN Program Manager Job Requirements: ·        Education: Associate's degree in Nursing required                   Bachelor’s degree in nursing preferred ·        Licensure: Wisconsin Registered Nurse/RN license required. ·        Experience: Minimum of four years’ experience in human service delivery with at least two years’ experience frail elderly individuals with dementia. A minimum of 2 years of leadership experience including direct staff supervision also required.   Registered Nurse/RN Program Manager Job Schedule: ·         Full time primarily weekdays ·         Manageable on-call rotation   Community Care is a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible.
