The Bridge Care Coordinator ensures timely and compassionate coordination of services for children with developmental disabilities and their families. This role provides a critical link between families, schools, and community resources to ensure that each child's unique needs are met through comprehensive and collaborative care planning.

About the Role:
The Bridge Care Coordinator ensures timely and compassionate coordination of services for children with developmental disabilities and their families. This role provides a critical link between families, schools, and community resources to ensure that each child's unique needs are met through comprehensive and collaborative care planning.
Responsibilities:
- Provides ongoing support and assistance to children/families on assigned caseload to access needed medical, educational, social or other services to address their children's needs.
- Helps families coordinate multiple community-based services and develop a coordinated plan to address assessed needs.
- Assures that the interests and preferences of the children/families are honored and respected when developing and modifying the Care Plan.
- Provides emotional support and advocacy to children/families on caseload.
- Provides support, assistance, and referral as identified and directed by the child and his/her family.
- Documents contacts, visits, concerns, and coordination in electronic medical record in a timely manner.
- Bachelor's degree in related filed preferred; equivalent experience may be considered.
- Eligible for Qualified Developmental Disabilities Professional (QDDP) status.
- Previous experience in development disabilities preferred.
- Ability to work with others in a positive, responsible manner.
- Strong writing and organizational skills in order to receive and convey information in efficient and effective manner.
- Able to flex schedule as needed.
- Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
- *Successful completion of a criminal history background check after hire.
