Community Care of the Lower Cape Fear, Inc is hiring a Housing Navigation Care Manager.
Job Purpose
The Housing Navigation Care Manager (CM) facilitates the Housing Navigation, Support, and Sustaining Services service line and other housing support services in the Healthy Opportunities Pilot (HOP). The CM has an integral role in the design, development, and implementation of care management activities as outlined in the HOP service description including but not limited to completion of screenings, assessments, and care planning to achieve goals that pertain to housing needs. This Housing Navigation CM works under the supervision of the Team Lead and in conjunction with CCLCF leadership.
Essential Duties and Responsibilities
· Conduct outreach, engagement, enrollment of referred services, and general risk assessment of member housing needs to initiate appropriate interventions.
· Assist with identifying and selecting housing preferences based off needs as well as connecting the member to services to support medical care needs related to housing including assisting to complete reasonable accommodation requests.
· Assist member with completing housing applications, obtaining necessary documentation and paperwork, and other materials needed for housing services.
· Develop goals, housing support and crisis plan as well as long-term housing stability plan,, budget, and other described support and educational activities for sustainable housing (i.e. budget skills development, financial literacy, independent living skills, etc).
· Coordinate housing supportive services including but not limited to housing inspections, housing move-in services, remediation and accessibility services, utility assistance, home goods, payments for security deposits and rent, referrals for legal supports, and transportation within HOP and outside of HOP.
· Connect member to education/training on tenants’ and landlords’ role, rights and responsibilities and assist member in reducing risk of eviction with conflict resolution skills.
· Complete thorough and timely documentation of all interactions, goals, plans, etc. in designated HIPAA- compliant documentation system.
· Provide effective, timely, and appropriate care navigation at all phases of planning process from receiving referral to outreach, screening, planning, educating, and ongoing evaluation.
· Provide ongoing care navigation services in the areas of employment linkage, resource navigation, benefits establishment, and other services as needed to assist in reaching goals.
· Encourage and promote an environment that is person-centered, inclusive, and culturally sensitive to assist members in meeting their individual goals.
· Collaborate with HOP Network Lead, Medicaid Prepaid Health Plans (PHPs), community agencies, coalitions, other HSOs, external care managers, and more.
· Perform other duties as assigned.
Qualifications
- Bachelor’s degree in social work or other related health services field or equivalent experience that provides the knowledge, skills and abilities to perform the job
- Minimum two (2) years of relevant work experience including with vulnerable populations (e.g. those who are chronically homeless, have substance use disorder, etc.), or equivalent and mutual empowerment for people in similar situations
- Reside in Bladen, Brunswick, Columbus, New Hanover, Pender, or Onslow county
- Valid NC Driver’s license with current auto liability insurance
- Excellent communication skills – oral and written
- Proficient Motivational Interviewing skills
- Strong organizational and time management skills
- Sensitivity to diversity of cultures, language barriers, health literacy and educational levels
- Knowledge of medical terminology
- Experience with Microsoft Office Suite, desktop publishing software, and virtual meeting platforms
- Must have received or be willing to receive the influenza vaccination by date of hire (during flu season only) to be considered. Proof of vaccination is required.
Physical Requirements
- May be required to sit for extended periods of time, lift objects up to 20 pounds on an occasional basis and do repetitive tasks such as typing on a regular basis.
- Multiple contacts are required with various members, providers, multi-payer systems and community partners to ensure coordination of services, therefore, exposure to general office and household conditions, as well as communicable disease could occur.
