PRIMARY FUNCTION: The Primary Care LSW, CSW, or LCSW in the school-based health center (SBHC) provides behavioral health interventions to treat the chronic and acute psychosocial, emotional, and behavioral aspects of children, teenagers, and their families, with occasional care as well to adult patients being treated by our OB/GYN and Adult Medicine departments in a primary care behavioral health (PCBH) model. This clinician conducts screenings and intake assessments, as well as individual, family, and group psychotherapy for mental and behavioral health and substance use needs by applying evidence-based practices in accordance with the applicable professional, ethical, and legal standards.
POSITION REQUIREMENTS:
- Training and experience in child and family mental and behavioral health.
- Ability to provide behavioral health consultations, care coordination, and consultation primarily with the school-based health clinic and pediatrics.
- Competency in delivering short-term and long-term evidence-based interventions.
- Effective use of screening and outcome tools to assess a range of mood and behavioral needs, including ADHD and suicidality.
- Advanced interpersonal skills to promote and participate in team-based care, patient-centered medical home model, trauma-informed care, the primary care behavioral health model, and SAMHSA levels 5-6 of healthcare integration.
- Knowledge of basic physiology, psychopharmacology, and medical terminology.
- Ability to provide excellent, timely clinical documentation within an Electronic Health Record.
- Competency in cultural awareness of the populations we serve and works from a frame of cultural humility.
- Adaptive to the opportunities and challenges of living and working in a rural setting on an island.
- A willingness to learn and demonstrate the principles of trauma-informed care and practice in all aspects of patient/client care.
ESSENTIAL DUTIES AND FUNCTIONS
Clinical Social Workers are core members of interdisciplinary primary care teams and part of our Integrated Health Department, providing care to all members of our community. Our clinic brings together a group of passionate professionals to provide whole-person care to our pediatric, adult, and OB/GYN patients. The patient care process includes:
- Promote and integrate behavioral health services within the organization and the clinical setting;
- Operate in a trauma-informed framework with both patients and coworkers;
- Conduct patient intakes, focusing on screening and functional evaluations, and make recommendations to the school-based health/primary care provider and/or parents concerning the patient’s behavioral, emotional, and/or medical treatment goals and plan;
- Provide brief, focused interventions and psychoeducation for patients and their families to address behavioral health needs (e.g. mental health, substance use, family and classroom dynamics);
- Provide primary care consultation and intervention for patients presenting in primary care with acute behavioral health needs (e.g. active suicidality, self-harm);
- Provide psychoeducation classes on a variety of topics (e.g. mood management, sleep hygiene, study skills);
- Conduct case management activities as necessary;
- Monitor and coordinate the delivery of behavioral health services for patients, including linking with other services and/or treatment providers in the community;
- Document and communicate about patients’ diagnostic information and treatment progress in the electronic medical record;
- Provide consultation to other team members about behavioral services and suggested areas of outcome and program evaluation;
- Support Quality Assurance activities related to behavioral health care;
- Function as a member of a multi-disciplinary team;
- Attend meetings, trainings, and other work-related events as needed.
- Ethics and Law
- Demonstrates a strong commitment to adhering to ethics, with special attention to dual relationships, confidentiality, informed consent, limits of competence, and team functioning
- Follows state laws, especially with regard to abuse reporting, adolescent reproductive health, and determination of decision making capacity
- Timely documentation submission- No more than 24 hours from the patient encounter.

QUALIFICATIONS
Education: Master’s Degree in Social Work from an accredited school of social work with intent to complete licensing for a Licensed Clinical Social Worker (LCSW) if not already obtained. Paid preceptorship available for individuals seeking LCSW.
Clinical Experience/Skills:
- Education and training as a clinical social worker.
- Training and experience in providing short-term and long-term treatments (e. g. CBT, solution-focused, problem-solving, motivational interviewing) with a focus on pediatric populations, addressing relevant mental health, substance-use, and physical health needs. Experience in primary care, medical settings, or working in the Patient-Centered Medical Home (PCMH) model preferred.
- Licensed in the State of Hawai’i (LSW/LCSW), or license eligible with expectation of licensure within 90 days of hire.
- Bilingual in English and Spanish, or English and Tagalog, Ilocano, or other Asian or Pacific Island language preferred.
- Collaborate and be an effective team member.
- Ability to multi-task, manage competing demands well, and complete clinical documentation in a timely manner.
- Maintain ethical and professional standards.
- Ability to effectively represent CCM in the community and maintain effective working relationships among co-workers, public, private and professional groups.
- Knowledgeable of community resources, including mental health, parenting, job training, substance abuse treatment and support groups, and Hawaii and other Asian and Pacific Islander cultural resources that support successful community integration and resilience.
- Valid driver license, reliable transportation, and proof of current vehicle liability insurance is required.
- TB clearance within the past 12 months.
