This position is in our Adult Medicine Department.
PRIMARY FUNCTION:
The Registered Nurse Team Manager is to provide leadership to the interdisciplinary clinical team in collaboration with Director of Clinical Operations. This position is required to be a champion for new models of care and strategic program, as defined by health center leadership and industry partners, facilitating change and coaching staff toward fulfillment of meeting these requirements. Such models and programs include but are not limited to Patient Centered Medical Home certification, Meaningful Use standards, and third-payer incentive programs.
This position serves as a liaison between the clinical team and other managers and administration by facilitating communication, overseeing orientation of new team members in collaboration with Human Resources and the Director of Clinical Operations and Quality Management and evaluating clinical performance with a focus on quality and performance improvement.
The RN Team Manager provides daily operational management, staff scheduling, performance management and risk management in collaboration with the Director of Clinical Operations and Quality Management. She/he is responsible for implementing operational and administrative goals established by senior management.
This position is key to the clinical teams operating in an efficient, safe and patient centered manner and providing the nursing staff with support and education regarding new processes and policies that effect daily operations.
The RN Team Manager provides direct patient care and case management as appropriate to insure optimal outcomes.
This position reports to the Clinical Operations Director.
Essential Responsibilities:
Clinical Operations:
- Responsible for day to day patient care activities including team meetings to plan and adjust patient flow and nursing assignments and team huddles.
- Facilitates and monitors day to day staff schedules and patient care assignments to meet efficiency goals and quality patient care.
- Collaborates with providers and practice staff in identifying appropriate patients for care management, utilizing established Care Management criteria.
- Identifies and effectively utilizes community resources to meet the needs of patients/families. Facilitates patient access to community resources as appropriate or refers to IH.
- Monitors work flows to improve patient check in, rooming, provider examination, education, and discharge.
- Monitors and directs the RN Case Managers to support patient education, self- management goal development, referrals, and appointments with specialists and outside entities in an effort to improve patient outcomes.
- Develops, implements and monitors staff schedules and patient care assignments in an efficient and cost effective manner.
- Mentors and role models appropriate customer service behaviors.
- Provides patient care as needed including but not limited to starting IVs, giving injections, working up patients, obtaining patient history, calling patients and performing case management.
- Evaluates the nursing and medical care provided to patients and their families by reviewing documentation in the EMR and observing interaction of staff with patients and family.
- Communicates with patients regarding complaints or concerns.
- Assess and respond to patients per telephone or in person.
- Maintains inventory of supplies and medications per policy and procedure.
- Ensures outdated medications are removed from the shelves and shipped for destruction.
- Ensures that staff completes laboratory quality controls are completed daily.
- Performs medication reconciliation for all care transitions.
Collaborates with CHW to understand the grants/programs related to clinical.
Management and Supervision:
- Interviews, hires, evaluate, counsels and recommends termination of staff according to policy and procedure.
- Evaluates competencies and develops educational recommendations for the nursing staff and team members.
- Collaborates with the Patient Care Team including behavioral health, registered dietician, tobacco cessation specialist, referral specialist, member services, dentists and medical to provide patients /family with needed services or to initiate referrals in the community.
- Contributes to the clinic’s financial objectives through the effective management and utilizations of resources.
- Monitors staffs’ compliance with infection control and OSHA standards, HIPAA regulations and confidentiality.
- Implements new policies, procedures, protocols and programs to meet the clinics requirements for retaining their FQHC status.
- Participates in the implementation of performance improvement objectives.
- Contributes to risk management objectives through the timely completion of incident reports, and the creation and implementation of a follow-up plan.
- Reviews utilization and quality reports routinely and scans for gaps in care to identify patients needing the additional support of care management.
- Provides coverage across the department as needed.
- Designs and evaluates processes to improve systems and patient care results across the continuum of care
- Performs all duties and responsibilities in accordance with the Nurse Practice Act and in NCQA, Federal, State, and local requirements in accordance with basic principles and guidelines of professional nursing.
- Monitors financial performance and identifies and implements strategies to reduce costs and improve quality of care/service.
- Provides ongoing staff development and training.
