Job Summary
Community Clinic is seeking a full-time bilingual Behavioral Health Case Manager to provide holistic and comprehensive case management services to its patients. The Case Manager works closely with a patient’s care team, including the provider, nurses, medical assistants, and behavioral health consultants. Key responsibilities include care coordination, working referrals, case management, progress monitoring, education, and advocacy. Bilingual proficiency (Spanish/English) is required. A Bachelor’s degree in social work or related field is preferred. Excellent benefits package including: insurance, 403(b), PTO and paid holidays.
Responsibilities
- Works within a team based approach to assist with coordinating continuity of patient care between Primary Care and Behavioral Health
- Manages care transitions after ER and inpatient hospital discharge, to facilitate timely engagement with Primary Care Provider.
- Coordinates and facilitates communication between patient and families, clinical provider and care team, Behavioral Health Consultant and referral sources (when applicable).
- Facilitates preventive wellness services and improvement in patient health outcomes, utilizing population health tools and reports.
- Conducts Behavioral Health screenings / assessments with patients and families when appropriate.
- Collaborates with clinical and other case management staff in the development and execution of the plan of care and achievements of goals.
- Assists the provider in understanding the history and physical or behavioral information from the patient, as well as the symptoms and reason for the patient’s visit.
- Interprets for Behavioral Health Providers, patients, and others when necessary and when language skills allow.
- Documents in the patient record in EMR.
Qualifications
- High School diploma or equivalent required.
- Bilingual English/Spanish required
- College degree in related field or related experience preferred.
- Strong interpersonal skills and the ability to work effectively with people of all backgrounds.

Pay: $17.00 - $18.00 per hour
Expected hours: 40 per week
