Independent Living Case Manager - Full Time Available

What you'll do as a Independent Living Case Manager:

• Work Monday through Friday 8am-4pm
• Provide support to 8-9 individuals both in their home and within the community.
• Utilize your personal vehicle to offer transportation services.
• Provide role modeling, training and direct care services encompassing all aspects of daily living
• Aid individuals in managing state issued benefits and government programs.
• Foster participation in individuals’ social and recreational activities tailored to individual interests.
• Assist in completing the ISP process for DDS funded individuals.
• Respond to the emergency needs of individuals.
• Oversee financial budgets, balance check books, bank deposit, timely payment of bills of persons on assigned caseload.

What you'll need to be a Independent Living Case Manager:

• High School Diploma or General Education Degree (GED) required
• Valid driver's license, access to a motor vehicle, and satisfactory driving record
• Strong written and verbal communication skills
• Strong computer and word processing skills
• 1 years’ experience and/or training working with individuals with disabilities preferred.
• 1 years’ experience in case management preferred.
