What are the responsibilities and job description for the Program Coordinator/Social Recreational Services position at COMMUNITY CONNECTIONS INC?
Community Connections Inc. is looking for a Program Coordinator/Social Recreational Services to join our team based in South Yarmouth, MA. In this role, you will support individuals with disabilities by providing direct support and organizing social and recreational activities that enhance their lives. You will partner with Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. We are looking for a Program Coordinator with a commitment to compassion, teamwork, and serving the community.
What You'll Do as a Program Coordinator:
- Assess clients’ interests, needs, and goals
- Develop, plan, and schedule a variety of social and recreational programs
- Encourage the acquisition of social skills and independence
- Demonstrate a high regard for everyone’s cultural values, expectations, and goals
- Work collaboratively with CCI leadership and State agencies
- Monitor members’ progress and response to program offerings
- Maintain case files and records to measure and track interests and progress
- Counsel and mentor members on building and nurturing meaningful friendships
- Maintain a member caseload and document progress
- Ensure the health, safety, and general well-being of the individuals
- Provide and maintain community-based habilitation/therapeutic experiences
What You'll Need as a Program Coordinator:
- At least one year’s experience in services for persons with disabilities
- Bachelor’s degree in a relevant human service field preferred (experience may substitute)
- Experience leading and facilitating small group discussions and planning activities
- Valid driver’s license and satisfactory driving record
- Acceptable criminal background check required
- Excellent written and verbal communication skills
- Demonstrated ability to work independently and as part of a team
- Strong organizational skills
