Job Description
Job Summary:
This position is full time consisting of 40 hours a week. Family Support Advocates promote the concept
of mental & physical wellness through interactions with your direct supervisor and other staff at CCFC
and the community. The goal is to help reduce/eliminate barriers a family may be facing.
Qualifications:
- High School Diploma or GED required.
- NYS Peer Certification prior to working in the position.
- Reliable and trustworthy, sound decision-making skills.
- Strong verbal, written and organizational skills.
- Strong active listening skills.
- Strong computer skills (Microsoft Word, Excel).
- Demonstrated ability to serve individuals and assist interagency and outside agency employees
in a professional, welcoming, and efficient manner.
- Cooperative, friendly, and helpful attitude with individuals and co-workers. Ability to work
closely with other employees to ensure a positive, constructive environment within individual
programs and overall agency.
- Ability to work independently and in a team setting.
- Ability to maintain confidentiality regarding the agency’s functions and individuals receiving
services.
- Must be able to practice professional boundaries with individuals you are working with, fellow
employee and partner’s employees always.
- Reliable transportation and a valid driver’s license, registration, inspection, and insurance that is
always active.
- Ability to engage and reengage individuals for services.
- Must be willing to self-disclose personal recovery.
- Knowledge of trauma and its effect on an individual.
- Must positively promote the agency within the community.
- Must be detail oriented.
- Must be able to work to multi-task in a fast-paced environment.
- Must be able to clear NYS Justice background check, NYS Office of Children and Family Services
background check and clear the NYS staff exclusion list, OIG, and HHS-OIG.
- Must have a clean driving abstract for the past 5 years.
- Must be willing to travel 75% or more per week.
Job Responsibilities:
- Must complete Mandated Reporting and HIPPA Training within first week of employment.
- Must obtain Family Peer Advocate Certification within 6 months of date of hire.
- Must obtain additional CEUs annually after receipt Family Support Advocate Certification to
maintain credentials.
- Must abide by all agency policies and procedures, along with school policies and procedures.
- Must adhere to professional boundaries with fellow employees, individuals receiving services,
and partner agency employees.
- Help coordinate transportation for medical, behavioral health, substance use and any legal
appointments.
- Must continually learn systematic processes for all systems in Franklin County such as, but not
limited to, housing assistance, food banks, courts, probation, Department of Social Services,
Public Defenders, Conflict Defenders.
- Must maintain confidentiality of families worked with and agency functions.
- Engage in supervision meetings with direct supervisor.
- Help to connect families to providers they deem suitable for their overall well-being.
- Must be able to determine and focus on root cause of issues, not just the issue presented.
- Maintain a professional working relationship with all agencies on behalf of CCFC.
- Must complete intake on the same day of service delivery.
- Must complete all progress notes in the TREAT system within 24 hours of delivery a service.
- Must submit monthly stats to direct supervisor by due date.
- Review social determinants of health with every family to determine what may be having a
negative impact on their overall health.
- Complete daily person-centered progress notes for families you are working with the same day
the service is delivered.
- Empower families you are working with through information and education at a level they can
comprehend.
- Attend required meetings and or trainings requested by the agency and or partner agency.
- Work with and educate all systems on the services offered at CCFC.
- Follow up with families to ensure they were able to connect with providers.
- Devise a plan to remove all barriers to connecting to services.
- Advocate for civil and human rights of families
- Any other job duties as assigned by your direct supervisor.
