This position is paid hourly with benefits. Longevity is rewarded. $18.91/hr for 0-3 years, $19.96/hr for 3-5 years and $21.53/hr for 6 years based on tenure with Community Connections. Title: Case Management Assistant
Reports To: Vice President of Case Management
FLSA: Hourly/Non-Exempt Full-Time
Job Description: Performs administrative support duties for Community Connection’s Case Management Agency.
Responsibilities:
- Faxes paperwork, PMIPs, CDASS and IHSS paperwork to medical professionals for signature, completes tracking and enters into state prescribed system. Calls after2 weeks to follow up if paperwork is not received.
- Processes, enters and uploads all PMIPs and attestations into the CCM
- Acquisition of home health care plans and medical records.
- Electronic file review, sorting and cleanup.
- Oversee legal files for all people in services and on the waitlist (Core files).
- Waitlist Management.
- Human Rights Committee (HRC) preparation, meeting attendance and follow up.
- Maintains provider contact list.
- Creates and updates forms for case managers, including converting documents to fillable PDFs.
- Connects clients to community resources and maintains the community resource folder.
- Maintains updated database and spreadsheets for member information, monitoring compliance, caseload list and statistics needed for funders.
- Processes enrollments, updates and terminations on case assignment list, SharePoint and On Target. Uploads documents in the state prescribed system.
- Pulls utilization, assists with service plan distributions and monitoring reminders.
- Meeting notes (support review meetings, case management meetings, HRC, etc.) and scheduling.
- Pulls files for audits and special projects.
- Manages the main phone line.
- Manages the shared fax mailboxes.
- Creates and prints labels and prepares mass mailings.
- Provides technical support to the case management team.
- Complies with all local agency Policies and Procedures and state and federal statute and regulations.
- Interacts in a professional and compassionate manner with all persons in services, families and community members.

Physical Demands of the Job: These essential tasks may be performed with or without accommodation or assistive technology. Employees in this position may spend significant hours on the computer writing and corresponding. The employee must be able to read text on the computer and paper documents. Communicating with others via email, letters and telephone is required. An employee in this position will use a laptop computer and may also have documents and files that they will need to transport within and outside the office. The employee will be required to attend meetings, training, and other community activities within the region of Southwest Colorado, with the possibility of occasional travel within Colorado or out of state to training or conferences.
