Job Description:
The Community Cooperative Case Manager supports our mission of ending hunger and homelessness in Southwest Florida by working with the Social Services and Education Team. Qualified candidates will have strong communication, organizational, interpersonal skills, and knowledge of working within an office environment. This position maintains positive, collaborative relationships inside and outside the agency while providing support to clients and program staff. The primary duties of the case worker position are to provide support to individuals and families, helping them access resources, navigate systems, and develop coping skills, while working collaboratively to create individualized plans that meet their needs and improve their well-being and other self-sufficiency skills.
Responsibilities:
- Develop individualized plans including identification of existing barriers to self-sufficiency, setting housing and income goals, and developing action plans.
- Make referrals to appropriate providers.
- Coordinate and advocate for the involvement of appropriate professionals from other agencies.
- Meet with clients on a regularly scheduled basis to discuss, assess, direct, and assist in the removal of barriers to self-sufficiency.
- Provide case management and follow-up services.
- Participate in and contribute to team.
- Maintain accurate and on-going documentation in individual client files, record-keeping forms and other reports.
- Follow contract requirements, agency, and department policies.
- Attend agency, department and networking meetings as required.
- Create, coordinate or facilitate educational classes to help clients improve credit scores, increase savings, and reduce debt.
- Other tasks and responsibilities as assigned and required by program guidelines.
Education, Training, Experience and Requirements:
- Bachelor's degree in a relevant field or equivalent case management experience.
- Strong organizational skills, attention to detail and excellent written & verbal communication skills.
- Ability to set priorities & manage a complex workload.
- Ability to work independently and as part of a team in a fast-paced environment.
- Ability to work and collaborate with a broad array of community partners & team members.
- Flexibility to work occasional evenings and weekends for events and meetings
- Dependable personal transportation to drive to sites.
- This position requires level 2 fingerprinting and pre-employment background screening.
- Must be able to pass background check, have valid driver’s license, and work in a drug free environment
Job Type: Full-time
