What are the responsibilities and job description for the Child and Family Case Manager position at Community Cooperative?
Responsibilities
- Develop individualized family/housing plan including identification of existing barriers to self-sufficiency, setting housing and income goals and developing an action.
- Make referrals to appropriate providers.
- Coordinate and advocate for the involvement of appropriate professionals from other agencies
- Meet with clients on a regularly scheduled basis to discuss, assess, direct and assist in the removal of barriers to self-sufficiency.
- Maintain accurate and on-going documentation in individual client files, record-keeping forms and other reports.
- Follow contract requirements, agency and department policies.
- Provide case management and follow-up services.
- Participate in and contribute to team.
- Attend agency, department and networking meetings as required.
- Create and facilitate educational classes to help clients improve credit scores, increase savings, and reduce debt.
- Other tasks and responsibilities as assigned and required by program guidelines
Education, Training, Experience and Requirements
- Bachelor’s degree in Social Work, Counseling, or related field with demonstrated experience
- Ability to set priorities & manage a complex workload
- Ability to work and collaborate with a broad array of community partners & team members
- Ability to work limited and periodic evening or weekend hours for special events
- Ability to work in a Drug Free Environment
- Dependable personal transportation to drive to sites
- Valid Florida Driver’s License
- This position requires a level 2 fingerprinting and pre-employment background screening.
Job Type: Full-time
Pay: From $18.00 per hour
Expected hours: 40 per week
