Greater Nashua Mental Health is looking for a full-time CTI Specialist to join our Critical Time Intervention Team! The CTI Specialist supports clients in critical transition from hospital level care into the community, ensuring connection to services such as mental health, primary care, substance use and developmental services over a 9-month period in an attempt to reduce utilization of the emergency room and hospitals.
Duties & Responsibilities :
- Engages with clients in the community with case management and functional support needs to ensure connection to primary care, mental health, substance use and / or developmental services over a 9 month period, in order to reduce readmissions and utilization of the emergency room.
- Documents and reports engagements with and care of clients.
- Meets clients in the hospital for pre-CTI (Critical Time Intervention).
- Educates and supports referral sources in order to make appropriate referrals.
- Supports program for fidelity requirements.
- Attends CTI team meetings, statewide consultation meetings and trainings as necessary.
