Description
Greater Nashua Mental Health is looking for a Therapist to join our Child, Adolescent & Family Services team! The Therapist provides clinically appropriate, evidence-based treatment and interventions, and case management services, to children, adolescents and their families diagnosed with mental illness. Services are provided in the office, the client's home, or in the community and school settings. Duties & Responsibilities :
- Provides individual, group, and family psychotherapy, as well as case management services, to clients for a percentage of time per week as appropriate for position and as directed by supervisor.
- Coordinates clients' treatment plan both with other GNMH staff members and outside agencies.
- Screens and assesses clients and makes appropriate referrals to a specific CAFS program and / or other agencies or services when the client is not an appropriate referral to CAFS.
- Completes all treatment plans, Child Needs and Strength Assessments (CANS), and Case Management Care Plans (CMCP) by due date listed in EMR.
- Performs other duties or special projects as required or as assigned.
- Performs all activities in accordance with ethical standards of the American Counseling Association.
