Case Manager - Youth Day Treatment
Location: Cape Girardeau, MO - Youth Day Treatment
Full Time: Non-Exempt
Hours: Monday - Friday 7:30am - 4:00pm. Some evening hours may be required dependent upon agency and client need.
Base Pay: $19/hour
The Program
The Youth Day Treatment program provides services to youth in grades 3-12 who require a step-down or an alternative to inpatient care or partial hospitalization, or as transitional care following an inpatient or partial hospitalization stay in order to facilitate return to the community or to prevent or minimize the need for more intense or restrictive level of treatment. The Day Treatment Program provides services that are more intensive than outpatient treatment and will serve persons who need a structured, behavioral health setting for daytime activities.
The Job
As a Case Worker, you would be responsible for the delivery of community support services for assigned clients enrolled in Youth Day Treatment services. These activities include coordination of delivery of mental health services, assessment and monitoring of client adjustment, resource development and linkage with community resources, providing crisis response to reduce risk of hospitalization of clients in crisis.
Essential Functions
- Provide at a minimum weekly services for clients that are enrolled in Youth Day Treatment services.
- Complete documentation required for referral to other programs as needed.
- Develop and monitor treatment plan goals and objectives and maintain contact with treatment team in regards to progress and concerns.
- Develop and implement individual service plans including crisis management plan.
- Identify services and resources to prevent hospitalization and out of home placement of client.
- Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
- Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
- Provide emergency in home conflict resolution including providing access to emergency services as needed.
- Intervene in behavioral health crisis situations, when needed, which may include use of seclusion and/or restrain interventions to prevent self-harm or harm to others.
- Based on need, provide seclusion and/or restraint interventions in accordance with Pro-Act guidelines and agency policy and procedure.
- Maintain certification in Pro-ACT and CPR/First Aid.
Minimum Required Qualifications

Bachelor's Degree in a Human Services field (see below),or;
- Social Work, Psychology, Nursing, Criminal Justice, Recreational Therapy, Human Development and Family Studies, Counseling, Child Development, Gerontology, Sociology, Human Services, Behavioral Science, and Rehabilitation Counseling.
- An individual with any four year combination of higher education and qualifying experience, or;
- An individual with any four year degree and two years of qualifying experience, or;
- An individual with four years of qualifying experience, or;
- Master's Degree in a human services field
Qualifying experience must include delivery of service to individuals with mental illness, substance abuse disorders, or developmental disabilities.
- Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.
- Travel in own vehicle is required.
Staff must have proof of continuous auto insurance, must be able to possess a MO Class E license, and successfully complete the required CPR/First Aid and Pro-ACT Training.
