Description of Position
We are seeking a full-time Care Coordinator to join our CBHC team in assisting our clients with mental health struggles. The ideal candidate is interested in providing a compassionate and full continuum of care to clients under the umbrella of the CBHC. You will be part of a multidisciplinary team of addressing the social, emotional, psychiatric, and medical needs of the clients we serve; this includes clients across the lifespan with a variety of presentations, including mental health and substance use concerns. Important qualities to be successful in this role include being self-motivated, good personal boundaries, and a belief in the principles of hope and recovery from mental illness. Responsibilities Include:
- Coordinating medical and behavioral health care and making referrals
- Completing referrals
- Working to identify, mitigate, and lift social and health inequities and barriers and advocate for the individual's access to services.
- Maintaining regular contact with client, provider, family, social service providers, and natural community supports to ensure treatment plans are implemented.
- Providing education and developing safety plans with clients and families.
- Obtain and maintain an active certification as a Mass Health Certified Application Counselor (CAC)
- Flexible Service Options: Offer counseling via telehealth and in-person sessions.
- Diverse Clinical Services: Individual and Group psychotherapy, wide range of populations, treatment planning, and use of standardized outcome measures.
- Crisis Support: Engage clients in brief crisis counseling and individualized risk management/safety planning.
- Collaborative Team: Work with a multidisciplinary team, including psychiatrists, care coordinators, recovery coaches, peer support specialists, nurses, and more.
- Must hold a Bachelor's degree in a mental health field.
- Must possess good organizational skills and effective written, verbal, and computer skills.
