Description
Job Duties :
Service Delivery Expectations :
1. Participates in the service delivery of day treatment programs as assigned.
2. Maintains client confidentiality at all times.
3. Provides day treatment services in compliance with DMH standards.
Documentation and Quality Improvement :
4. Provides input for the development of weekly summary progress notes and monthly reviews of service plan objectives. Documents relevant events when instructed to do so by the day treatment specialist or program therapist.
5. Completes and submits service logs per agency policy when assigned by day treatment specialist.
6. Assists treatment team members with prior authorization process as requested.
Interpersonal and Leadership Skills :
7. Attends and participates appropriately in clinical staffings when requested to do so by the day treatment specialist.
8. Responds to clients needs in an appropriate and timely manner.
9. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.
Support Duties :
10. Completes additional job duties assigned by supervisor.
11. Participates in interagency collaboration for the provision of client services. Serves as liaison with local school systems to promote day treatment services.
12. Notifies an appropriate designated supervisor a minimum of two hours in advance of any program absence so that the supervisor can arrange program coverage if necessary.
13. When utilizing a CCS vehicle : Signs out vehicles in compliance with CCS policy. Completes inspection prior to driving. Maintains safe driving habits. Ensures cleanliness of vehicles.
Key Responsibilities
Requires excellent interpersonal skills.
Requires satisfactory completion of CPR prior to reporting to the work site.
Requires satisfactory completion of CPI within 30 days of employment.
Requires valid driver's license and auto liability insurance.
Requires ability to be covered under company insurance to drive a company vehicle for client transportation.
Requires ability to transport clients in private vehicle when necessary.
Requires completion of annual training in compliance with DMH standards.
Previous experience in a mental health setting strongly preferred.
Previous experience working with an SED population in a treatment setting strongly preferred.
