About the Role

The job of case manager/nurse is a clinical, direct service position. The job consists of case management services to Community Counseling Services’ clients that are served through a distinct program – IMPACT. The IMPACT program serves adult individuals identified as having severe and persistent mental illnesses and their families.

Community Counseling Services expects employees to be welcoming to consumers, including those with co-occurring disorders, and engaging them at their current stage of recovery. Services provided will be in accordance with the plans and guidelines as established by Community Counseling Services and the State Division of Behavioral Health. Portray and encourage civility to all clients, customers, employees, community partners and vendors.

Requirements

Education and Experience
  • Possession of a minimum of an LPN, bachelor’s or master’s degree in nursing or other health-related field is highly desirable.
Certificates, licenses, registrations
  • Minimum of LPN, RN preferred.
  • Valid South Dakota Driver’s license.
  • Maintain licensure status.
  • Skills, knowledge, and/or experience that demonstrates an ability to deal appropriately and in a caring manner with individuals with severe and persistent illnesses.
  • Must not be excluded from participating in Medicare, Medicaid, and any Federal health care program, as provided by the OIG, USDHHS.

Other qualifications
  • Ability to work effectively in a multi-disciplinary environment.
  • A strong commitment to working as an integral part of a team.
  • Must have a valid driver’s license or the ability to obtain one and be
willing to transport consumers

ESSENTIAL DUTIES/CRITICAL JOB ELEMENTS

Case Management

Provide case management services to consumers and families as defined by the Division of Behavioral Health and Community Counseling Services and will include:
Client Care
  • Conduct client intake, assessments and diagnostic interviews
  • Individual and group skills building with clients and family members.
  • Develop and update treatment plans in a timely manner.
  • Documentation of all client related contacts reflecting individual treatment plan.
  • Complete progress reports and discharge summaries in a timely manner.
  • Referral to appropriate services to meet client needs.
  • Participate in and utilize treatment teams concepts.
  • Case management and liaison services with family and referral sources.
  • Participate in staff meetings;
  • Provide on-call crisis intervention on a rotating schedule to include evenings, weekends, and holidays.
  • Complete outcome tool as directed by Clinical Supervisor
  • Complete Division required surveys as directed by Clinical Supervisor
  • Provide ongoing assessment, problems solving, skills training, supervision and environmental adaptations to assist the consumer in performing basic daily living activities including (but not limited to):
a. Medical, dental and vision services;
b. Support in helping consumers find and maintain employment in community-based job sites;
c. Budgeting and financial support including payee services through the provider agency, apply for entitlements, money management and accessing financial services;
d. Linking to social service agencies;
e. Legal advocacy and representation;
f. Support consumers with performing personal hygiene and grooming tasks
g. Performing household chores, including housecleaning, laundry, and shopping for household items and cleaning supplies;
h. Provide nutrition education, meal planning, grocery shopping and food preparation.
i. Teaching and role modeling appropriate and effective parenting techniques;
j. Educating consumers and families regarding mental illness and recovery;
k. Teaching and role modeling appropriate social skills and emotion regulation in a variety of situations;
l. Accessing transportation – obtaining driver’s license, insurance and car, arrange for cabs, access community transportation or find rides;
m. Locating, financing and maintaining safe, clean affordable housing; helping with apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities (e.g., telephone, furnishing, linens)
n. Consult with the family to ensure quality of care, problem solve and mediation;
o. Interpersonal relationship encouragement, support, mediation and role-modeling healthy relationships;
p. Collaboration with substance abuse services as appropriate.

  • Direct all nursing activities with the IMPACT team in providing medication administration and medical services, as directed by the Nursing Director and Medical Director.
    • Give injections as prescribed
    • Maintain and organize Medication Administration Records (MARS)
    • Utilize E-Scribing System
    • Assist Prescribers as needed.
    • Implement Meaningful Use objectives as directed by Nursing Director
    • Assist with State and indigent funding applications for medication
  • Other duties as assigned.
  • A full time provider will work 40 hours a week.
Licensure & Credentialing
  • Maintain professional licensure
  • Participate with credentialing staff to establish and maintain billing/admitting/service credentials
  • If eligible, gain QMHP status.
