What are the responsibilities and job description for the Family Preservation Specialist position at Community County Services, Inc?
HIRING BONUS $750 after 90 days of employment! DESCRIPTION: Family Preservation Services practices evidence-based principles with certified training based on Evidence Based principles through the National Family Preservation Network (NFPN), The Institute for Family Development and Adoption and Safe Families Act (ASFA) Safety Assessment and Management Process. It is a three to six-month intensive program that provides intervention to youth and families identified at imminent risk of placement out of their home or transitioning home from placement. At the end of three months if determined by CYS/JPO/Court that the family has made satisfactory progress and is not yet able to be maintained with other community-based services an extension with court recommendations for an additional three months of services is allowed. The model's efficacy will be maintained with the National Family Preservation Network (NFPN).
TRUANCY services are designed to follow evidence based interventions that are designed to alleviate familial pathology that prevents children from getting to school. These issues may be related to poverty, mental health of either the parents or the child, bullying, etc. We work to alleviate whatever the underlying issues are to maximize the child's opportunity for a quality education.
PROGRAM CHARACTERISTICS:
1. Intensive Family Preservation services have immediate 24-hour contact with up to 72-hour response to the referral.
2. Individual therapy caseload with an option for team delivery for clinical necessity.
3. Provision of services at highly flexible times(7 days a week 24 hours/day) to meet the needs of the children/youth and family, court, and CYFS.
4. Provide services primarily in the home and other natural environments.
5. Carry small caseloads of 6 to 12 families (and additional case when prior cases are transitioning out) in a three-month period. Providing intensive intervention of 20 to 30 hours/month.
6. Provide CRISIS and on-call coverage and intervention (24/7).
7. Provide Comprehensive Assessment.
8. Deliver intensive individually designed Goal Oriented Intervention, safety planning & stabilizing basic needs, positive reinforcement, behavioral/cognitive skill training, problem-solving with critical thinking, strength-based approaches, motivational strategies, self-efficacy and communication skills, and teaching strategies empowering victims of abuse/neglect with cross-system planning including roles and responsibilities of each participant to prevent duplication.
9. Team approach w/ CYFS/JPO, and other specified services or persons.
10. Provide concrete services and linkage to the specifically qualified level of care w/ appropriate funding streams to match the family/child resources and insurances.
11. Function as a bridge between the family, CYFS, /JPO court, school and community resulting in optimum planning, support and outcomes.
