Record entries on flow sheets, notes, and charts when applicable.
Report significant changes in the resident’s condition to an LVN / RN as soon as practical.
Do not disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to your supervisor.
Monitor a new resident’s room is ready for the resident, i.e., the bed is made, name tag in place, ect.
Introduce yourself, roommates, and other staff as appropriate to new residents.
Mark and inventory the resident’s personal possessions as instructed.
Assist with transferring residents as necessary.
Perform assigned duties in accordance with state, federal, and facility policies and procedures.
Follow work schedules / assignments in performing and completing your assigned tasks.
Cooperate with all facility personnel. Report problems to your supervisor.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment.
Report complaints and grievances made by the resident to appropriate personnel.
Notify your supervisor when you will be late or absent from work.
Report occupational incidents, including near misses.
Assist residents with daily oral care, i.e., brushing teeth / dentures, oral hygiene, etc., as directed.
Assist residents with bath functions, i.e., bed bath, tub or shower bath, sponge bath, etc., as directed.
Assist residents with dressing / undressing as needed.
Assist residents with grooming, i.e., hair shampooing / brushing, nail care for non-diabetics, shaving, ect.
Change bed linens as needed or as schedules.
Check and report bowel movements and stool condition as instructed.
Assist residents in preparing for medical tests / appointments, activities / social programs, and with transporting to / from.
Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, ect.
Assist residents to walk with or without self-help devices as instructed.
Answer resident calls as promptly as feasible. Assure that residents who are unable to call are checked regularly.
Observe and report the presence of pressure areas and skin breakdown.
Report all injuries including those of unknown cause, including skin tears.
Turn bedfast residents as least every two hours.
Turn over all medication found in resident’s room to the Nurse Supervisor / Charge, Nurse.
Report environmental changes in room temperature, ventilation, lighting, etc.
Prepare residents for meals, i.e., take to the bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to / from dining room etc.
Serve food trays and assist with feeding as indicated.
Record resident’s food / fluid intake as instructed. Report changes in residents’ eating habits.
Perform after meal care, i.e., remove trays, clean resident’s face, hands, clothing, take to the bathroom, etc.
Keep residents’ water pitchers clean and filled with fresh water and within easy reach.
Serve between meal and bedtime snacks.
Check rooms for food articles, i.e., food in proper containers, unauthorized food items, etc.
Check rooms for unauthorized medications.
Attend and participate in all scheduled training, educational and inservice classes to keep current with changes / issues and to maintain certificate.
Restrain residents only as instructed by a nursing supervisor. Release residents at least every two hours.
Wash hands before and after performing any service for a resident.
Keep the call button within easy reach of the resident.
Report any hazardous condition / equipment to the Nurse Supervisor / Charge Nurse immediately.
Follow established isolation precautions and procedures. Wash hands before entering and after leaving an isolation area.
Report any communicable or infectious disease to the Director of Nursing and / or the Infection Control Coordinator.
Wear and use safety equipment and supplies, i.e., back brace, gait belt, mechanical lift, etc. when lifting or transferring residents.
Use only the equipment that you have been trained to use, operating the equipment in a safe manner.
Report any defective equipment.
Treat residents fairly, and with kindness, dignity and respect.
Knock before entering a residents’ room and assure all nursing care is provided in privacy.
Report all allegations of resident abuse and / or misappropriation of resident property.
Honor and report the resident’s refusal of treatment request.
Perform other duties as deemed appropriate by your supervisors.
SPECIFIC REQUIREMENTS :
Must be a currently certified as a Certified Nursing Assistant in accordance with the laws of this state.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to lift, push or pull 50 lbs. Also, must possess the ability to bend, kneel or stoop.
