Job Overview
We are seeking a dedicated and compassionate CFSS consultation Case Manager who speaks English and Spanish to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary support and services tailored to their unique needs. This position is essential in navigating complex healthcare systems, particularly within a Level I or Level II Trauma Center environment. The Case Manager will work closely with patients, families, and multidisciplinary teams to facilitate optimal health outcomes.
Duties
- Conduct comprehensive assessments of patients' needs, including medical history and social factors affecting their care.
- Develop and implement individualized care plans in collaboration with healthcare providers and patients.
- Coordinate services across various disciplines, ensuring continuity of care throughout the treatment process.
- Maintain accurate and up-to-date medical records while adhering to confidentiality standards.
- Provide education and resources to patients and families regarding treatment options, community resources, and support services.
- Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
- Advocate for patients’ needs within the healthcare system, including addressing barriers to care.
- Collaborate with social workers, addiction counselors, and other professionals to address psychosocial aspects of patient care.
- Stay informed about medical terminology and current best practices in case management.
Experience
Candidates should possess the following qualifications:
- Experience in case management or social work is preferred, particularly within a medical or trauma center setting.
- Familiarity with medical records management and understanding of medical terminology is essential.
- Background in pediatrics or addiction counseling is a plus.
- Strong communication skills with the ability to work effectively with diverse populations.
