Job Title: WOMEN’S REENTRY DOMESTIC VIOLENCE CASE MANAGER II

Location: Women’s ReEntry Services Program at Women’s Resource Center

Reports to: Director of Women’s Services

Status:  Non-exempt, Union

Wage: $29.00/hour  

Hours: Monday - Friday, 9am-5pm

We focus on the hardest to reach, most vulnerable people: Women, the TGNC Community, Medical Crisis, Substance Use Disorder, and Older Adults. Our programs meet the needs of people, rather than demanding that people meet the needs of our programs.

JOB SUMMARY:

Community Forward SF is hiring a Domestic Violence Case Manager II for our Women’s ReEntry Services Program. The Case Manager will provide trauma-informed, culturally responsive, and strengths-based services to address the needs of female-identified incarcerated survivors of domestic violence (DV), stalking, and trafficking. These services will include group facilitation, individual case management, crisis counseling, referrals to community-based organizations and legal services, and coordination of treatment and safety plans for released clients.  These services will be provided to clients in custody as well as at the Women’s Resource Center (WRC). 

The Women’s ReEntry Services program will also incorporate Community Forward SF’s Mental Health Outpatient Program by enrolling clients, conducting assessments, creating treatment plans, providing mental health support, and crisis intervention. Case management services offered will include but are not limited to: substance use disorder referrals, conflict resolution, mediation, benefits counseling, advocacy, employment services, and in-home support services via outside providers, such as medical counseling and access to food pantry programs.

ESSENTIAL JOB FUNCTIONS:

  • Establish and maintain a caseload of clients appropriate to size of program, provide services including, but not limited to, stabilization support, re-entry counseling,  individual counseling, crisis intervention, assessment, case management, substance abuse support, and advocacy.

  • Facilitate three weekly groups at WRC, County Jail 2B Pod, and County Jail 2 Upper A Pod: Seeking Safety, Mindfulness for Beginners, and a mindful nutrition and eating group.    

  • Develop treatment plans with timelines for individual goals and objectives.

  • Meet with clients on a weekly basis to ensure successful engagement with and completion of treatment plan.

  • Ensure access to resources that help stabilize clients’ income through benefits advocacy, housing, medical status, and other needs, including social service systems and other agencies.

  • Conduct outreach to encourage program usage and maintenance of program census.

  • Coordinate enrolling clients in Community Forward SF Mental Health Outpatient Program or other appropriate programs based on client need.

  • Maintain client charts and associated documentation in a timely manner by successfully and effectively utilizing City electronic health record systems that may include Avatar and other justice-involved database systems as well as successful use of G-Suites applications.

  • Submit client progress notes and updates to the clinical supervisor on a weekly basis.

  • Meet with the clinical supervisor on a monthly basis and present client cases during monthly Group Supervision meetings.

  • Assist clients in attending appointments with outside service providers. 

  • Coordinate care with client service providers to ensure quality service and continuity of care.

  • Assist clients in developing external support systems.

  • Attend to needs of crisis clients as needed.

  • Adhere to established policies and procedures, specific to program protocols as well as adhere to compliance standards and participate in audit preparation activities

  • Facilitate meetings with other providers to ensure continuity of care

  • Ensure that clients are treated with dignity and respect, and that their physical needs are adequately met while residing at the facility.  

  • Assist Women’s Resource Center with administrative support duties as needed. 

  • Support our mission to promote stabilization, hope, and healing for adults experiencing homelessness in San Francisco. 

  • Mandatory training and meeting attendance.

  • Other duties as assigned.

BACKGROUND & EXPERIENCE: 

  • Bachelor’s (B.A.) degree or equivalent in education and work experience.

  • Previous Domestic Violence case management experience and/or training. 

  • Current Domestic Violence 40-hour certification.

  • Comfort with technology including basic knowledge of Microsoft Office Suite, Google Suite, and city housing databases and electronic health record systems, including but not limited to Avatar, other justice-involved database systems, and the ONE Coordinated Entry system.

  • Possess advanced knowledge of the justice system and the needs of re-entry clients.

  • Possess ability to write clear and clinically-relevant client progress notes, treatment plans, assessments, and associated clinical reports.

  • Possesses knowledge of DSM-V diagnoses and classifications. Possess knowledge of clinical orientations, modalities and interventions.

  • Possess effective communication skills and the ability to work both independently and cooperatively.

  • Possess ability to receive jail clearance within 30 days of hire.

  • Possess ability to complete Seeking Safety certification within 90 days of hire. 

  • Ability to de-escalate conflict and provide crisis interventions

  • Knowledge of homelessness, healthcare, substance abuse, HIV, harm reduction, co-occurring disorders, dementia/cognitive impairment, cultural diversity issues and relevant resources.

  • Able to work effectively with diverse individuals who self-identify by cultural, language, neighborhood, sexual identity, gender and other definitions.

  • Ability to write and speak English effectively.

  • Ability to provide own transportation 

  • CPR and first aid certification required within 90 days of hire.

  • 5150 Trained & Certified within the first year of hire or promotion. 

PREFERRED QUALIFICATIONS:

  • Bicultural, bilingual Spanish speaking strongly preferred.

  • Current jail clearance

  • Current Seeking Safety certification 

  • Knowledge of San Francisco community resources, 

  • Experience working with at-risk client populations.

  • Potential to work/be on-call on evenings and/or weekends.
