What are the responsibilities and job description for the Intake Specialist position at Community Guidance Center?
- Conduct intake evaluations and triages as assigned that will include preparing all required documentation, administering and scoring assessment tools, complete an individualized service plan based on the consumers identified needs and wishes, complete Psychosocial Evaluation, and present the case in an efficient and coherent manner at the daily staffing conferences using social learning.
- Handle incoming requests for services by collecting needed information from the referral source, determining what services are needed, facilitate appropriate and timely referral to provide these services, complete the required documentation for the intervention, prepare Releases of Information as indicated, provide crisis intervention as needed and schedule appointments when appropriate utilizing social responsibility.
- Attend Intake department meetings, staffing conferences and individual supervision as scheduled and other meetings relevant to maintaining sound clinical case management. Maintain case notes, service plans, case revision, authorizations and relevant paperwork requirements to insure complete, ethical, uninterrupted clinical care for the consumers and their families exercising social responsibility.
- Maintain all documentation and fiscal standards. Maintain complete files and clinical documentation. Comply with quality assurance guidelines. Maintain awareness of all Federal, State and Managed Care laws, policies, regulations, and procedures regarding clinical care, Best Practice guidelines, and appropriate fiscal requirements related to clinical services.
- Conduct appropriate consultation with the treatment team regarding clinical care and policies/procedures utilizing social learning. Conduct external consultation with other community providers, schools and agencies as indicated. Attend regular supervision with the immediate supervisor and openly communicate all issues which impact quality of clinical care.
