Summary
We are seeking a Discharge Coordinator/Case Manager to work with our forensic team at the Butler County Jail to assist individuals transitioning from incarceration back to the community with a specific focus on addressing mental health and substance use treatment and recovery supports. The ideal candidate will be responsible for planning, coordinating and facilitating the transition of incarcerated individuals from jail to the community. This role involves assessing individuals post release needs, connecting them with essential resources and collaborating with community agencies to ensure a smooth reintegration process.
Job Duties and Responsibilities
- Assist clients in obtaining services through community resources by making appropriate referrals.
- Completes individualized discharge plans for inmates identified as needing follow up care.
- Collaborates with jail staff, probation officers, courts, case managers, and other stakeholders.
- Assists forensic team members with responding to inmate needs relayed via the jail inmate communication system.
- Coordinates inmate assessments and referrals to jail contract psychiatrist and manages follow up needs as determined by visit with prescriber.
- Provides advocacy and assistance with accessing the health care system.
- Participates in care coordination activities with representatives of other agencies; provide reporting as needed or requested by community providers.
- Maintains accurate, complete, concise, and timely documentation in the electronic medical record; that represents provided services.
- Tracks post-release outcomes and follow up.
- Provide and/or arrange for crisis intervention for clients in a crisis.
- Establishes a rapport, maintains clear professional boundaries, and focuses on assignment/task completion.
- Complies with all agency policies and procedures; promotes professional behaviors, dress, and interactions with clients, community stakeholders, and staff.
Qualifications
Qualifications:
- Minimum of high school diploma, prefer bachelor's degree in social science field.
- Experience in case management, corrections, social services or reentry programs.
- Strong knowledge of community resources and social service programs.
- Familiarity with the criminal justice system.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work effectively in a jail environment and with justice-involved individuals.
- Operate a personal computer and other office equipment.
