JOIN OUR TEAM of dedicated professionals in fostering hope and transforming lives.  We are seeking a compassionate organized and recovery-focused Case Manager to join our team.  This role supports individuals who are in a residential substance use treatment program by coordinating access to treatment, recovery supports services, and essential community resources to promote stability, engagement in care and sustained recovery.

Job Duties and Responsibilities:

  • Conducts initial and ongoing needs assessments and assist clients in identifying recovery goals. 
  • Coordinates and links clients to appropriate treatment services, primary care, housing, employment, legal, social and recovery supports.
  • Monitor client progress towards treatment plan goals. 
  • Advocate on behalf of clients and support engagement in recovery-oriented and person-centered services. 
  • Ensure accurate and timely documentation of all services in compliance with agency policy and regulatory standards.
  • Participates in case conferences, team meetings and trainings as required. 
  • Aids in the development of individualized service plans in collaboration with the client and the interdisciplinary team.
  • Maintains strong clinical and ethical boundaries with clients.

Qualifications


  • High school diploma or equivalent. 
  • Associates or Bachelor’s degree in Psychology, Counseling, or a related Human Service Field preferred.
  • CDC-A certification preferred.
  • Valid driver's license and insurable driving record. Position requires community based work.
  • Minimum of one year of experience working with individuals with mental health or substance use disorders.
  • Strong interpersonal and communication skills. 
  • Ability to work independently and as part of a multidisciplinary team. 
  • Experience with writing clinical documentation. 
  • Familiarity with trauma-informed care, harm reduction and motivational interviewing techniques.  
  • Strong knowledge of community resources and systems. 
  • Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams.
  • Adept at learning and navigating electronic health record systems.
