Summary
As a Outreach Specialist for individuals experiencing homelessness, you will be responsible for engaging with individuals experiencing homelessness, assessing their needs, and connecting them with mental health or substance use treatment services and other resources and supports. Your role involves building trusting relationships, advocating for clients, and facilitating access to resources to promote housing stability and improve quality of life.

Job Duties and Responsibilities
- Conduct regular outreach in the community to identify and build relationships with individuals experiencing homelessness.
- Engage with clients in a compassionate and respectful manner to assess their needs, build trust and encourage participation in services.
- Conduct assessment of needs to identify barriers to housing stability and prioritize client needs.
- Coordinate comprehensive behavioral health assessment with the agency’s assessment teams.
- Develop individualized service plans and goals in collaboration with clients to address behavioral health, healthcare, employment, housing and other essential needs.
- Connect clients with appropriate resources and services, including emergency shelter, transitional housing, healthcare providers, mental health and substance use treatment and social services.
- Advocate for clients to ensure access to entitlements, benefits and community-based supports.
- Provide crisis intervention and supports to clients experiencing mental health crises, substance use issues, or other emergencies.
- Collaborate with crisis response teams, law enforcement, emergency medical services as needed to ensure client safety and well-being.
- Build partnerships and collaborate with community organizations, homeless shelters, healthcare providers, law enforcement, and local government agencies to coordinate services and support for individuals experiencing homelessness.
- Participate in community meetings, task forces and committees focused on homelessness prevention and solutions.
- Maintain accurate and up to date documentation of client interactions, assessments, service plans, referral and outcomes in accordance with agency policies and regulatory requirements.
- Prepare reports and data analysis to track program outcomes, measure impact and support funding applications.
- Works with clients and staff in an ethical, culturally sensitive, and respectful manner.
- Establishes a rapport, maintains clear professional boundaries, and focuses on assignment/task completion.
- Complies with all agency policies and procedures; promotes professional behaviors, dress, and interactions with residents, community stakeholders, and staff.

Education and Experience Requirements
- Minimum requirements: High school diploma.
- Preferred: Associate’s degree or experience in behavioral health industry.
Certification, Licensure and/or Registrations Required
- Valid driver's license and auto insurance
- Qualified Mental Health Specialist (QMHS) designation upon hire
