Job Summary : The Community Health Alliance (CHA) Community Health Worker (CHW) is dedicated to improving patient outcomes related to social determinants of health, improving access to resources, assisting patients in controlling and preventing chronic conditions. Community Health Workers are certified trained public health educators improving health care delivery requiring integrated and coordinated services across the continuum of health under the supervision of their collaborative practice with a medical provider.
