Position Summary:

A Health Care Coordinator (HCC) helps CHA Members and their families achieve recovery by assessing needs, identifying service opportunities, and encouraging goal-setting. Key responsibilities include intensive case management, coordinating integrated care services, and providing direct support in areas such as employment, health promotion, motivational interviewing, skills training, and group facilitation. HCCs deliver care as directed by the Clinical Director/Supervisor, respecting member rights and cultural differences while promoting dignity, independence, and choice. They create and refine treatment plans based on ongoing assessments and facilitate services like daily living support, psychosocial rehabilitation, and community integration. Additionally, HCCs provide positive reinforcement, skills training in communication and relationship-building, and monitor members for safety, preventing crises and relapses. They engage with members during community and home visits to foster recovery and self-sufficiency.

Primary Duties/Responsibilities:

· Provide timely outpatient support services per Individual Service Plans, which may include but limited to case management, supportive counseling, skills training, crisis intervention and service coordination, and document these services using DAP format in the electronic health record.

· Complete Core Behavioral Health Assessments and updates, documenting needs in compliance with agency standards.

· Develop and regularly update individualized treatment plans based on assessed needs and member progress.

· Meet key performance indicators and production standards for the caseload.

· Coordinate discharge for members which may include but not limited to coordinating safe transportation for members to necessary appointments through AHCCCS or own vehicle.

· Refer members to appropriate care levels, monitor services per DBHS/RBHA criteria, and document treatment outcomes within an Adult Recovery Team Meeting/ Child and Family Team Meeting setting.

· Assist members with scheduling and following up on physical health care services and screenings.

· Conducts home and community visits, attends team meetings, and travels locally using a personal vehicle.

· Coordinate services with Managers, Clinical Supervisors, and local agencies, including law enforcement and Probation Departments.

· Complete additional assessments (DLA-20, ASAM) to determine care levels and monitor progress.

· Coordinate services with Managers, Clinical Supervisors, and local agencies, including law enforcement and Probation Departments.

· Comply with Community Health Associates policies and regulatory requirements, including HIPAA.

· Interact professionally with colleagues, members, and community agency staff.

· Provide care that respects member rights, cultural differences, and promotes dignity and independence.

· Attend clinical supervision meetings 1 hour every other week or more if recommended by the clinical supervisor.

· Perform special assignments and other duties as assigned.

Minimum Qualification:

High school diploma or GED required; bachelor’s degree in behavioral health preferred, or a combination of four years of relevant experience and education. Must be able to obtain a Level One Fingerprint Clearance Card. Reliable transportation, clean driving record, Arizona driver’s license, and current personal automobile insurance required. Bilingual Preferred

Skills:

Excellent communication skills with a patient, objective, and nonjudgmental attitude. Strong problem-solving abilities and knowledge of community resources and agency services. Team player with good interpersonal skills and a positive attitude. Ability to follow clinical and office protocols, multi-task, and use computers effectively. Knowledge of electronic health record keeping systems. Basic knowledge of counseling modalities such as motivational interviewing and CBT. Knowledge of HIPAA and healthcare regulations.
