Primary Role:
The Medical Case Manager (MCM) will provide services under the supervision and direction of Population Health Manager. The MCM will provide client-centered services and implement the clinical treatment plan, including linking clients into medical, mental, and dental healthcare and other needed services. The coordination and follow-up of medical treatments are key components of medical case management. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the clients, and other key family members’, needs and personal support systems. Medical case management includes the provision of treatment adherence counseling to ensure readiness for and adherence to complex HIV/AIDS treatments.
Qualifications
Qualifications:
- Education: Bachelor’s Degree in social work, psychology, public health, or related field
- Experience:
- Minimum 3 years case management experience
- One year of relevant experience may be substituted for each year of required education.
- Minimum two years’ experience working with target population, especially services to
- HIV-positive, homeless, and/or mentally disabled individuals, preferably chronically homeless populations
- Completion of Ryan White Medical Case Management Training (within 3 mo. after hire)
- Demonstrated ability to utilize computers and demonstrated proficiency in Microsoft Office
- Experience using a data management system; CAREWare experience preferred
- Employment Eligibility Verification, including updated tuberculosis (T.B.) test and COVID vaccination; and successful completion of background screening
- Strong written and verbal communication skills
- Ability to work with diverse communities

D. Physical Requirements: May require periods of sitting for long periods of time. Will require travel and some out of the area and overnight travel may be expected. Strong communication and interpersonal skills are a hallmark, and you are adept working across a variety of teams, from providers to operations leaders and executive leadership.
Essential Job Duties:
- Conduct bio-psychosocial assessment of each client receiving case management services twice per year
- Develop an individual service care plan for each client (ongoing)
- Determine client eligibility for Ryan White services and a broad range of auxiliary services
- Assist clients in applying for services and benefits
- Coordinate, monitor, and follow-up on needed client services
- Conduct home visits, medical visits, and other community visits with clients
- Provide treatment adherence assistance
- Ensure service delivery aligns with policy, regulation, and case management standards
- Ensure service data and case notes are entered within 2 business days
- Ensure client charts are in good order for state audits
- Attend and participate in the state Case Management required trainings
- Assist eligible and appropriate clients who may benefit from enrolling into the 340B program
- Develop individual professional development goals annually
- Reinforce treatment goals that clients develop with medical care providers
- Document linkage and navigation data from client encounters
- Collaborate with staff across CCHN, community-based HIV service providers and testing organizations and medical providers to support clients’ needs
- Assist with reporting requirements including documenting data for reports, preparing reports and submitting data to funders
- Assist with agency-wide activities as directed, including Collaborative Meetings, NJ HIV Planning meetings, and others
- Assist with closing quality gaps for patients not accessing services
- Perform other duties as assigned
