**Job Summary:**
The Case Manager will provide case management, coordinate services, and counsel individuals to reduce dependence on emergency assistance programs by accessing entitlements, benefits, and community resources.
**Responsibilities:**
- Provide short-term counseling and crisis intervention services to clients.
- Assess client eligibility and help with the completion of forms for entitlements and supplemental insurance.
- Assist clients in accessing prescription cost assistance programs.
- Advocate for clients when engaging with community agencies.
- Maintain accurate client records and prepare necessary documentation.
- Coordinate services like transportation assistance and handle client correspondence.
- Recognize the need for client education and support services.
- Consult with Providers and make referrals when necessary.
- Evaluate and cater to the needs of special population clients.
- Offer counseling, testing services, and appropriate referrals.
- Follow up on missed appointments and assist with client management.
- Ensure a clean and safe work environment.
- Attend clinical and team meetings regularly.
- Perform any other relevant duties as assigned.
**Qualifications:**
- Bachelors Degree in Social Work or a related field.
- Two years of experience as a Case Manager with strong interpersonal and communication skills.
- Proficiency in MS Office and general computer literacy.
**Preferred Qualifications:**
- Five years of experience as a Case Manager.
- Experience working with diverse populations.
**Physical Demands:**
This position involves sitting, standing, walking, bending, and stooping. Some weekend and evening work may be required.
**Salary Status:**
Hourly; non-exempt.
Pay Range: $20 - $24/hr.
