PHYSICAL REQUIREMENTS

JOB SUMMARY
The Nurse Practitioner (NP) provides professional medical services in the community health center setting. Under the direction of the CMO, the NP provides comprehensive primary care services to all patients within the life cycle (zero to death). The NP collaborates with other members of the health care team in the management of patients to coordinate referral and follows-up care.
EDUCATION AND EXPERIENCE
Current Nurse Practitioner certification and eligible for Licensing in Iowa.
Demonstrates satisfactory knowledge and clinical judgment to provide appropriate care for clinic patients.
At least two years’ experience working with underserved or low-income persons preferred.
Ability to perform cross cultural health education and counseling for health problems required.
Bilingual English / Spanish highly preferable.
Demonstrates an understanding of and sensitivity to the health care issues facing low-income and underserved populations.
Qualifications
Must possess a valid Iowa NP license.
Board certified in family practice or other primary care specialty.
Must have interest and knowledge of current trends in health care, especially with regard to providing health care to low-income populations in a culturally sensitive manner.

RESPONSIBILITIES & ESSENTIAL POSITION FUNCTIONS
Performance of health assessments, including a thorough history and physical examination, ordering or performing certain diagnostic tests, evaluation of the psychological and family aspects of a situation sufficient to make a general health assessment and diagnose the nature of acute and chronic conditions.
Promotion and maintenance of adult and child health, including planning of periodic health assessments and screening for early case finding, assessment and age-appropriate guidance with regards to risk factors, family dynamics, good health habits, family planning, mental and emotional problems, and use of CHCFD as the medical home.
Management of acute and chronic illness, within the scope of practice. Including counseling and education, as well as performance of minor surgical procedures and initiation of life-saving emergency measures.
Supervision of staff including making recommendations for promotion, commendation, or disciplinary action when necessary or appropriate.
Establishes a collaborative relationship with other medical providers with referral of complex clinical cases to a physician in situations with complicated diagnostic problems or complicated therapeutic problems.
Participates in the development of protocols as needed.
Provide after hour call to be shared among other health care providers (NP, PA, MD / DO)
Complete charting and paperwork in a timely manner. This will also include Electronic Health Records as applicable.
Participates in continuing education courses, in services and staff meetings and attends other meetings as assigned.
Participates in Performance Improvement and Quality Review.
Productivity is satisfactory and consistent with the organizational goals.
Responsible for other related job duties as assigned by the CMO and CEO.
